SCOPE OF WORK:

The Bureau of Indian Affairs, Eastern Regional Office (BIA, ERO), requires a vendor to perform construction tasks in support of program operations provided by the BIA, ERO, Choctaw Agency. For further information on the region see: https://www.bia.gov/regional-offices/eastern .

To support this requirement the Department of Interior, Bureau of Indian Affairs, Eastern Regional Office will offer a single award for these construction tasks.

Respond to this solicitation with a quote for each project as a line item together in the table below.

Pricing shall note the cost of each project inclusively.

As a Buy Indian Act set aside you must certify in the DIAR section 1452.280-4 on page 10 for consideration.

PROJECT 1 (Statement of Work)

Choctaw Indian Reservation, Mississippi Lake Pushmataha Dam Intake Tower Ladder

1) LOCATIONS

Lake Pushmataha Dam is located at 32° 48' 10"N, 89° 15' 00"W, Pearl River Community, Mississippi Band of Choctaw Indians.

2) BACKGROUND

Lake Pushmataha Dam is located on Jones Creek approximately eight miles west of Philadelphia, Mississippi, on the Choctaw Indian Reservation. The Dam is an earthen embankment located in East- Central Mississippi. The Dam was constructed in 2003 by the Mississippi Band of Choctaw Indians to provide for enhanced aesthetics and recreation activities near the Pearl River Community, Mississippi.

Lake Pushmataha is approximately 4,633 acre-feet at its recreation pool water surface elevation (El.)

448.0, and 5,060 acre-feet of water at auxiliary spillway crest El. 451.0. The lake surface area is https://www.bia.gov/regional-offices/eastern https://www.ipp.gov/ approximately 285 acres. The Dam is a homogeneous Earthfill embankment dam, with a crest length of approximately 1,900 feet, a crest width of 40 feet, a structural height of 55.5 feet, and a crest El. 455.0.

The upstream slope of the dam is sloped at approximately 3 Horizontal to 1 Vertical (H:V) and is protected by an approximately 2- foot-thick layer of rock riprap underlain by a geotextile between El. 455 and El. 440.0. The upstream slope is unprotected below El. 440.0.

The downstream slope of the dam is covered by grass and some small brush and is approximately 3H:1V. The toe of the downstream slope is at about El. 421 and a stability berm is present beyond the downstream toe. The stability berm slopes gently (15.6H:1V) and extends to about 150 feet beyond the toe of the slope. The downstream slope of the dam contains a mid-slope shelf at approximately El. 428.

The shelf collects and directs runoff from the downstream slope of the dam to a concrete drain intake.

The drain intake also allows for personnel access and is located on the shelf. Drainage collected in the intake/manhole is conveyed through a horizontal, 24-inch-diameter corrugated metal pipe (CMP) that extends 274 feet and discharges in the right side of the energy dissipation structure. The CMP drain is aligned to the right of the principal spillway and outlet works conduit.

In 2019, a seepage berm was constructed on the right side of the embankment (from approximately Station 25+00 to Station 29+00). Based on record drawings, the seepage berm consists of American Society for Testing and Materials International (ASTM) C-33 sand, ASTM 67 stone, a 16-ounce (oz) geotextile, miscellaneous Earthfill, and a 12-inch high-density polyethylene (HDPE) seepage collection pipe. The sand zone is 3 feet thick (vertically), slopes 3 horizontal to 1 vertical (H:V) and contacts the original downstream dam slope. A 2-foot-thick (vertical) zone of ASTM 67 stone is downstream and adjacent to the zone of C-33 sand. The top of the sand and stone zones are at approximately El. 440 feet. The sand and stone zones are covered with 16-oz non-woven geotextile. The outer zone of the seepage berm consists of miscellaneous Earthfill also sloped at 3H:1V. Seepage collected within the berm is collected in a 12-inch diameter HDPE pipe which discharges into a downstream, riprap-lined channel. A principal spillway tower is located within the reservoir near the maximum section of the dam.

The principal spillway consists of an uncontrolled drop-inlet overflow. At El. 448.0, the uncontrolled discharges pass over 7.5-foot-long weirs on both the left and right sides of the structure, for a combined weir length of 15 feet. The tower is 2.5 feet by 7.5 feet. The tower structure also includes two slide gates that enable both high-level and low-level control of the reservoir water. The two gates are installed on the upstream and downstream walls of the tower. A 30-inch by 30-inch upward-opening slide gate on the upstream side of the tower allows for low-level reservoir releases through a 30-inch- diameter round opening with an invert El. 408.0. A second 30-inch by 30-inch downward-opening slide gate on the downstream side of the tower allows for reservoir releases and reservoir level control below just the principal spillway. The downward opening gate can allow for reservoir level regulation between El. 447.5 and El. 445.0. Both gates are operated from a concrete platform at the top of the tower (accessed only by boat). Flow that enters the tower (over the principal spillway weirs or through gate openings) is conveyed through the dam within a 520-foot-long, 30-inch-diameter reinforced concrete conduit to an impact-type energy dissipation structure located beyond the downstream side of the stability berm. The invert elevation of the conduit is El. 408.00 at the upstream end and El. 396.00 at its downstream end.

The concrete energy dissipation structure is 11 feet wide by 14 feet 9 inches long and 9 feet 11 inches high, at floor El. 394.17, with 8-foot 10-inch-high side walls that transition to 5-feet 5- inches high at the downstream end of the structure. The structure has a central baffle wall that dissipates the flow energy by impact.

Metal grating serves as the top platform for the energy dissipation structure. A 175-foot-long, riprap-lined trapezoidal discharge channel conveys outlet discharge from the energy dissipation structure to the main creek channel. The discharge capacity of the principal spillway is approximately 108 cubic feet per second (CFS) at reservoir water surface El. 451.0; however, others have previously stated that this may overestimate flow because of orifice control and other flow restrictions in the tower. The auxiliary spillway is located on the left abutment and consists of a trapezoidal earthen channel. The base of the channel is 150 feet wide, and the channel sides consist of 3H:1V slopes. The crest (invert) is at El. 451.0. The spillway channel is covered with grass. The discharge capacity of the auxiliary spillway is approximately 4,800 CFS at reservoir water surface El. 455.0. Due to the current reservoir restriction at the lake, access to the outlet works gates by boat is a hazardous condition. Currently, staff use an extension ladder tied to the tower to gain access and must climb over the open upper gate. Therefore, the BIA pursuing installation of a permanent ladder mounted to the tower to improve this condition.

3) PRE-SUBMITTALS

Included in the contractor proposal, the following Pre-submittals are required:

Schedule-A schedule detailing the execution of the work.

4) SCHEDULE AND PAYMENT

Contractor shall have thirty (45) days to complete assigned scope of work (from issuance of Notice to Proceed (NTP)).

Payment will be considered full compensation for all office and field services as well as all material(s) and expense(s) required to complete the contract.

Measurement and payment shall be in accordance with the bid schedule below. Payment for unit priced line items will be made at the agreed upon unit price.

The contractor shall notify the government when 100% of the contract quantity is completed.

6) SCOPE OF WORK

Provide all equipment, labor, and materials necessary to install a fixed safety ladder along with a fall arrest system as depicted in SKETCH LADDER-1. The ladder shall be installed on the upstream side of the intake tower and likely requires boat-based installation. The contractor shall verify all dimensions and make necessary adjustments for the installation. Two harnesses with all required accessories compatible with the system shall also be provided.

Any damages to the facility or surrounding areas shall be corrected by the contractor at no additional cost to the government. The contractor is responsible for compliance with Environmental Protection Agency requirements regarding the Clean Water Act and National Pollutant Discharge Elimination System.

The contractor is responsible for any permits, licenses, taxes, or other fees associated with the work on the Choctaw Indian Reservation.

7) SAFETY

Contractor shall implement an appropriate safety control program applicable to the work being performed in accordance with Federal and State laws. A Fall Protection Plan shall be instituted if workers are to be performing duties where their feet are greater than six (6’) feet from the ground or finished floor. All personnel on roadway projects shall wear appropriate Personal Protective Equipment (PPE), i.e.: hard hat, safety glasses, steel toed work boots, hearing protection and safety vest. Contractor shall bear sole responsibility of taking all appropriate actions necessary to prevent accidents and injuries to individuals, both contractor and general public, in, and or, around the work area.

8) SUBMITTALS

Product Data: Contactor shall submit Product Data for materials for government approval.

9) FINAL DELIVERABLES

Complete installation of safety ladder and fall arrest system.

PROJECT 2 (Statement of Work)

10) PROJECT INFORMATION:

Project Location: BIA Choctaw Agency, 421 Powell Street, Philadelphia, MS 39350

Project Description: BIA Choctaw Agency 56 window replacement consists of removing and disposing of old windows and replacing with new windows.

11) SCOPE OF WORK (SOW):

The contractor shall provide all personnel, equipment, tools, materials, labor, supervision and other items and services necessary to perform and complete the work described in this SOW, i.e., BIA Choctaw Agency window replacement. The project will consist of removing the current outdated fifty-six (56) different sized windows and replacing them with all new single pane, double-insulated vinyl windows. All work performed under this SOW shall be conducted in accordance with applicable state and federal laws and regulations.

12) DESCRIPTION OF WORK:

a) Contractor shall replace existing fifty-six (56) windows on the Choctaw Agency building with 56 new windows.

b) Contractor shall ensure that all new fifty-six (56) windows are vinyl, single pane, and double insulated that are all installed plumb, level, square and watertight.

c) Contractor shall ensure that all interior and exterior finishes match existing finishes.

d) Contractor shall replace with new material and matching color of existing finishes to any/all damage caused to interior or exterior during installation.

13) PERIOD OF PERFORMANCE:

Work must be completed within 120 days from contract award.

14) MATERIALS:

Fifty-six (56) new single-hung, double insulated vinyl windows.

Window sizes: Qty. 1 @ 76” x 58”, Qty. 13 @ 36” x 56”, Qty. 31 @ 38” x 68”, Qty. 3 @ 44” x 20”, and Qty. 8 @ 34” x 24”

CONTRACTOR RESPONSIBILITIES:

a) Contractor shall verify all site conditions, quantities, measurements, and project requirements.

b) To keep workers and the public from injury, the contractor shall protect and barricade work areas at all times.

c) Contractor is also responsible for the repair or replacement of any existing equipment, furniture, sheet rock, including painting of walls, etc. which may be damaged during this project. Finishes shall match existing.

d) Contractor shall provide a warranty on all Contractor provided material and labor for a period of two to five years (longer if standard for materials/equipment) from the date of acceptance by the POC.

15) UTILITIES:

The Agency shall provide at no cost to the Contractor reasonable amounts of water and electricity to support the contract. Hook up will be to existing electrical outlets and existing water shut off valves and will be the contractor’s responsibility. Any contractor requirement to connect to the base water system will need to be coordinated through the project manager and will require a backflow prevention device.

16) DISPOSAL OF MATERIALS AND CLEANUP:

The contractor shall provide the appropriate disposal bins/trash receptacles Approval for the location(s) of disposal bins/trash receptacles shall be coordinated through the Point of Contact (POC). The Contractor shall cleanup work site daily and dispose of all debris to an appropriate bin and/or receptacle. Contractor may not use any Agency/Government disposal bins, dumpsters or trash receptacles for any reason at any time.

17) GENERAL REQUIREMENTS:

a) Work hours Monday – Friday: 08:00 AM – 04:30 PM. Coordinate work hours with the

POC, Eric Nicky (629) 266-1942. If weekend work is required, Contractor shall submit a request to the POC for approval a minimum of 48 hours in advance. Contractor shall keep the POC updated on status of the work and shall inform the POC of projected completion.

b) Contractor must provide proof of insurance coverage upon acceptance of contract.

c) Contractor shall check in daily with the POC who will allow access to the Contractor into the building in which the work will be performed that day. Contractor shall follow the established scheduled workdays and work times unless otherwise scheduled and coordinated with POC.

PROJECT 3 (Statement of Work)

19) Project Information:

Project Description: Choctaw Agency Fire building (formerly old wood shop) will renovate and install new 14

ft. garage door, install new rain gutters and down spouts, and install new drainage culvert in front of newly installed garage door.

20) Scope of Work:

The contractor shall provide all services, materials, labor and installation required to perform the work described in this SOW, i.e. Choctaw Agency Fire Building, install new 14 foot garage door, remove and install new rain gutters, and install new drainage culvert in front of new installed garage door.

Work shall include but not limited to the following:

a) Install new Garage door:

i) Cut out existing sheet metal with owners’ discretion in framing for new 14x10 foot garage door, cut out and create opening with 8-inch C purlins and trim.

ii) Install and frame for 14x10’ Gray Raynor Duracoil Plolyurethane Insulated 24 gauge flat slat rolling steel service door with Raynor control hoist 2-1/2HP with operators.

iii) Run and Wire garage door from closet appropriate electrical source

b) Install new Rain gutters and down spouts:

i) Remove old rain gutters and down spouts on building.

ii) Install approximately 170 Linear feet of 24 gauge light grey prefinished commercial box gutters for both sides of building

iii) Install two down spouts approximately 15 feet long on each side of newly installed gutters.

c) Install new drainage culvert in front of new installed Garage door.

i) With direction from Site POC, Dig trench approximately 2-3 feet from concrete slab directly in front of newly installed garage door. Trench will be approximately 2-3 feet deep, with minimum 6 inches of gravel at bottom of trench.

ii) Lay 40-inch steel, .372 thick pipe, 12 feet in diameter in trench.

iii) Fill and pack trench, so BIA Road crew can lay approximately Temporary 4-6 inch gravel drive way over the newly installed pipe culvert

21) Contract Responsibilities:

a) Contractor shall verify all site conditions, quantities, measurements, and project requirements

b) Contractor shall be responsible for damaging any existing utilities while digging underground, Contractor must make sure lines are marked before digging.

c) Contractor shall provide system Operation and Maintenance, and other necessary documents for operation of installed garage door.

22) Utilities:

a) The BIA Choctaw Agency shall provide at no cost to the contractor reasonable amounts of water and electricity to support the contract. Hook up will be to existing electrical outlets and existing water shut off valves and will be the contractor’s responsibility.

23) Disposal of material and Cleanup:

The contractor with coordination from BIA Choctaw Agency shall provide disposal dumpsters, and will coordinate the placement of the dumpsters. The contractor shall cleanup work site daily and dispose of all debris to an appropriate bin or receptacle.

24) General Requirements:

a) Work hours: Monday-Friday 08:00 AM-04;30 PM. Coordinate work hours with POC, Mr. Eric Nickey.

If weekend work is required or after hours, the contractor shall coordinate with POC and request approval 48 hours in advance.

b) Contractor shall check in daily with POC who will allow access to the project site.

(a) The offeror represents as part of its offer that it [ ] does [ ] does not meet the definition of Indian Economic Enterprise (IEE) as defined in DIAR 1480.201 and that it intends to meet the definition of an IEE throughout the performance of the contract. The offeror must notify the contracting officer immediately in writing if there is any ownership change affecting compliance with this representation.

Payment requests must be submitted electronically through the U. S. Department of the Treasury's Invoice Processing Platform System (IPP).

"Payment request" means any request for contract financing payment or invoice payment by the Contractor.
