4. EQUIPMENT
Contractor will supply all the equipment and chemicals necessary to accomplish the work that is to be done under this Agreement.
5. PREVENTIVE PROCESS
A. Conduct a floor-level inspection of the facility, covering all five pest-activity zones; entry points, water sources, food sources, harborage points and employee areas.
B. Make available an analysis of the layout of the facility, noting all potential problem areas, both inside and out.
C. Ensure accurate pest identification, including identification of different kinds of pests as well as various growth stages.
D. Provide a sanitation consultation at each visit to help SDC keep key areas of the facility clean and unattractive to pests.
E. Devise an integrated pest management system and implement a treatment strategy conforming to the IPM system.
F. Formulate a customized treatment program to address any immediate pest problem.
G. Ongoing monitoring and maintenance, with treatments adjusted accordingly to maximize effectiveness. Each visit must include a written Service Report.
6. METHODS
A. Non-Chemical Techniques
1. Glue boards/sticky traps of varying types may be used to trap crawling insects and rodents for the dual purpose of monitoring and control. They will be positioned in areas most likely to be frequented by pests, yet placed so as not to interfere with operations. All placements must be inaccessible to residents and will be recorded on a detailed site diagram.
2. Pheromone traps may be deployed as needed to monitor insect populations.
3. Mechanical traps of various types may be used as needed to monitor and/or control rodents. Each station will be inaccessible to residents and mapped on a site diagram.
4. Vacuums may be used as needed to remove insects and any debris that might serve as an insect food source.
5. Foam sealant or caulking may be used to close openings that can be used by pests, either as harborage or entry points.
B. Chemical Techniques
1. Pesticides, on site staff and contracted pest management companies shall follow an Integrated Pest Management (IPM) strategy that focuses on long-term prevention or suppression of pest problems through a combination of techniques. Therefore, when establishing a pest treatment plan, properly licensed staff shall use non-chemical and biological controls. If this treatment is ineffective, use Tier 3 (least hazardous) herbicides/insecticides, progressing to Tier 2 and then to Tier 1 (most hazardous) only if necessary to manage the pests. Use only Tier-rated herbicides/insecticides as listed on the current San Francisco Department of Environment Hazard Screening List.
2. Rodent baits (if necessary) will be used in enclosed bait stations on exterior locations of the facility, such as the building perimeter and fence line. Bait stations in all exterior locations and other areas that are accessible to children, pets or non-target wildlife will be of a tamper-resistant variety.
7. SERVICE PROCEDURES
A. Exterior Locations
1. Insect and Crawling-Pest Control
Contractor will use baits or seasonal residual insecticide applications as needed in specific, targeted areas around the immediate exterior of the facility. Care will be exercised to prevent drift of materials into the facility or landing on surfaces that might allow them to be picked up on shoes or wheels of equipment and be transported inside the facility.
B. Interior Locations
1. Insect and Crawling-Pest Control
During each service visit, Contractor will inspect for conditions conducive to pest infestations and report any needed repairs or maintenance and sanitation issues. All traps will be inspected monthly and the information kept on an accumulative log sheet.
8. Contractor service documentation
A. Site Diagram: All pest control devices are numbered and indicated on the site diagram.
B. Inspection Reports: Details areas inspected, problems found and corrective recommendations, i.e., what SDC and Contractor need to do to fix the problem.
C. Service Reports: A Service Report will be completed at the end of every service. Pest activity and action plans to correct any pest problems will be documented, as well as structural and sanitation recommendations.
D. SDS/Labels: Safety Data Sheets, or labels, provide details about materials used in compliance with state and federal regulations and quality audits.
E. Contacts: Provides all pertinent contact information at your facility, including client requirements and service date and time, should the Commercial Pest Specialist need assistance or to communicate any problems in case of emergency.

SDC employees will report all pest sightings in a logbook for Contractor to review during each weekly visit.
10. TIME OF SERVICE
Contractor will provide services during the term of the contract on dates specified by the SDC Project Representative. The time of day required for the work to be done may be anytime during the 24 hours of any given day in order to provide the work during hours when the area to be serviced has the least amount of “traffic”. There will be no extra payment for any hour chosen regardless of the clock time.
11. EMERGENCY SERVICES
In the unlikely event that additional emergency services are necessary between regular visits, they will be provided and billed according to Exhibit B of the contract documents. Emergency services shall be performed within 24 hours of initial phone call to Contractor.
12. LICENSE
Contractors and workers shall be properly licensed: Current structural Pest Control Operator; Qualified Applicator License issued by the California Department of Pesticide Regulation. Contractor agrees that all services rendered under the terms of this agreement shall be subject to the approval of the State and that the State may terminate this agreement at any time upon giving a 30 day written notice to the Contractor.
13. LOSS OF LICENSE
This contract will terminate if at any time should Contractor lose their structural pest control operator license. This contract may be terminated at the discretion of the State if at any time the license of the Contractor is suspended.
