About this file

This is a Seller Checklist for Small Order Purchases issued by the City of Philadelphia Procurement Department through the PHLContracts platform, applicable to a micro purchase for Safety Blanket & Smock products related to suicide/isolation prevention. The checklist outlines the procedural requirements for vendors submitting quotes for this procurement opportunity in Pennsylvania. Sellers must complete all required steps sequentially, including selecting the appropriate seller role, answering the Local Business Entity (LBE) certification question (though the LBE Program does not apply to small order purchases), entering unit costs for all listed items, answering mandatory questions, and accepting the terms and conditions of the bid. The micro purchase will be awarded as a whole or by line item, whichever serves the City's best interest, to the lowest responsive and responsible bidder. Notably, sellers must quote all items to be eligible for award, and no delivery or services should commence until formal notification from the department.

The checklist requires vendors to submit an executed Consent & Authorization form as a mandatory attachment without which no quote will be considered, and permits optional submission of an LGBTQ Applicant Opportunity Data Form. All required attachments must be downloaded, saved, and submitted through the Attachments Tab, and sellers must complete a comprehensive review of their quotes on the Summary Tab to address any red validation warnings before final submission. The procurement process is conducted entirely through the PHLContracts online platform at www.PHLContracts.phila.gov, where sellers can access and monitor their quote submissions through the Quotes tab.
