About this file

This document is a Seller Checklist for Small Order Purchases issued by the City of Philadelphia Procurement Department through the PHLContracts platform. The checklist provides guidance for vendors submitting quotes for a micro purchase of barber chairs to be awarded to the lowest responsive and responsible bidder. The purchase may be awarded as a whole or by individual line items, at the City's discretion. The document does not specify quantities, response deadlines, due dates, site visits, or bidder conferences related to this particular procurement.

Vendors must complete a multi-step submission process including: confirming seller role status, indicating Local Business Entity (LBE) certification status (though the LBE Program does not apply to small order purchases), entering unit costs for each item, answering mandatory questions, accepting terms and conditions, and submitting required attachments. Critical requirements include providing an executed Consent & Authorization form in the vendor profile and completing any attachments specified in the bid. Vendors are encouraged to submit an optional LGBTQ Applicant Opportunity Data Form. The submission is completed through the PHLContracts.phila.gov portal, where vendors must address all validation warnings before submission. No specific pricing information, funding sources, contract terms, or renewal options are detailed in this checklist document.
