VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 Revision 02 Eff Date: 03/31/2020

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C249-20-AP-1523

1. Contracting Activity: Department of Veterans Affairs, VISN 9, 626 Chattanooga OPC, 1639 Medical

Center Parkway, Suite 400, Murfreesboro, TN 37129 Identify purchase request (i.e. 2237) number- 626-20-2-4164-0036

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Planmeca ProMax dental system and romexis software, installation and training, to be used for planning dental procedures and surgery.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

In accordance with 6.302-1(c), only one responsible source and no other supplies or services will satisfy agency requirements. The TVHS, Chattanooga OPC needs a 3D Cone Beam dental system compatible with the existing MIPACS dental picture archiving and storage system. The system must have the following specification: Windows 10, ability to transmit radiographs through VISTA, 3D imaging capability with stand-alone 2D imaging capability, software to perform surgical planning and surgical guides and facial photos and imaging overlay. TVHS is currently using Planmeca dental systems. All clinical dental and biomedical maintenance staff are trained on the Planmeca dental x-ray systems. Network interfacing already exists for the Planmeca dental system. In support of consistency within TVHS dental department, it is cost effective and most advantageous to the government that the clinical and biomedical staff continue to use the same equipment and supplies.

Thereby precluding consideration of a product manufactured by another company.

4. Description of market research conducted and results or statement why it was not conducted:

Market research revealed two or more verified SDVOSB/VOSB capable of providing dental equipment. This request is for the ProMax 3D dental equipment with Romexis software. The dental equipment and software are manufactured by Planmeca and is proprietary. However, Planmeca has verified SDVOSB authorized distributors or resellers that can provide the dental equipment and software.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
