This is a Single Source Justification for a federal procurement by the Department of Veterans Affairs.

Crescenz VA Medical Center in Philadelphia (VISN 04) is acquiring brand name accessories for an existing Leksell stereotactic system with an estimated contract value of $118,988.36. The accessories are necessary to shorten surgical preparation time, reduce risk of component loss during sterilization, improve outcome accuracy, and support modern advancements in deep brain stimulation procedures.
The contracting officer justified the single source award based on the unique compatibility requirements with the existing Leksell system and the absence of alternative compatible accessories. Market research conducted between May and June 2026 included posting a sources sought notice with no responses, querying the VIP database using NAICS Code 339112, and searching multiple government and commercial databases including GSA Advantage, GSA eLibrary, National Acquisition Center, and contractdirectory.gov. 1. Contracting Activity: Department of Veterans Affairs, VISN 04, Philadelphia VA Medical Center, 3900 Woodland Ave., Philadelphia, PA, 15240. Purchase request number 642-26-3-315-0113.
2. Brief description of supplies required and the intended use and estimated amount: The Corporal Michael J. Crescenz VA Medical Center in Philadelphia is requesting brand name accessories for their Leksell stereotacic system to enhance the performance of the system and the services the medical center provides through the Neuro-Surgery, Deep Brain Stimulation program. The Philadelphia VA Medical Center will be able to expand their surgical services. This system is used to help shorten preparation time and lessens the risk of losing parts during cleaning and sterilization. This system is used for diagnostic and therapeutic procedures providing accurate results with dedicated instrument and solid fixation.
Deep brain stimulation is a recognized treatment for various neurological conditions, including Parkinson’s disease, essential tremor, and dystonia. The procedure involves implanting electrodes within certain areas of the brain to produce electrical impulses that regulate abnormal impulses or affect certain cells and chemicals within the brain.
The Philadelphia VA uses the Leksell stereotactic system for precise navigation and targeting during these intricate procedures. While the existing system provides a solid foundation, also certain components must always have back-ups to maintain the highest standards of care, improve outcome accuracy, and expand the procedural capabilities in line with modern advancements in deep brain stimulation.
The total estimated contract value of the proposed action is $118,988.36
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: A Leksell stereotactic system is already in operation at the Philadelphia VA and these accessories will enhance performance and expand the program’s capabilities. There are no other known accessories that are compatible with the existing system. The highly specialized Leksell Vantage system was purchased (36C24421P0847) in September 2021. The Leskell system’s unique arc employs the center-of-arc principle for encompassing the surgical target in three dimensions, enabling full access to any intracranial area.
In addition, this principle enables limitless trajectories and entry points to be used.

4. Description of market research conducted and results or statement why it was not conducted: A sources sought was posted to System for Award Management on May 22, 2026 and closed on May 29,2026 with no responses. Identified awards made to both Elekta and Red One Medical (SDVOSB) on AddsUp. Reached out to Red One for an authorized distributor and have reasonable expectation that they will be able to provide an updated authorized distributor letter to submit with their quote.
The VIP database was queried on 06/04/2024 using the NAICS Code "339112" and keywords "Stereotactic" which did not provide any additional capable sources.
Searched National Acquisition Center, MedSurg Catalog Search GSA Advantage, GSA eLibrary, FPDS. There are no existing government contracts for these items. The requester queried the Government-wide database of contracts and other procurement instruments intended for use by multiple agencies available at www.contractdirectory.gov and other government and commercial databases such as US Spending that provide information relevant to agency acquisitions did not provide any additional results. None of these searches yielded any useful results, therefore the solicitation will be posted unrestricted, open market.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR 13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
