About this file

This document is an Emergency Purchase Statement from the State of Illinois' Chief Procurement Office related to a credit monitoring services contract with Equifax Consumer Services LLC.

The State of Illinois' Healthcare and Family Services agency purchased a one-year credit monitoring subscription for a subset of Medicaid providers affected by a recent data breach involving personally identifiable information. The original emergency contract had an expected start date of June 1, 2021 and an estimated end date of August 29, 2021 for a total of $342,200. The agency is now requesting a contract extension until December 31, 2021 to allow the providers to complete enrollment in the credit monitoring services and for the vendor to continue providing customer support during the enrollment period. There are no additional costs associated with the extension.

Emergency Purchase Details Reference #: 21-478HFS-GENCO-B-22734

Emergency Category (select the appropriate option from the drop-down menu below):

To prevent or minimize serious disruption in critical State services that affect health, safety, or collection of substantial State revenues

Reason for selection of this particular vendor:

The Office of General Counsel (OGC) contacted three different companies that could possibly supply these services. The Vendor was chosen because it was the only Vendor that offered a three-bureau credit monitoring package at a fixed price. The fixed price is needed to control costs. And we need increased speed in reviewing all three bureaus given the length of time that has transpired since the breach event as well as due to the sensitive nature of the information involved. Additionally, the Vendor's offer was the most cost effective.

Description of supplies or services to be provided:

The Vendor will provide credit monitoring services for a subset of Medicaid providers affected by a recent data breach involving personally identifiable information (PII). The contract will also include administrative services related to the credit monitoring such as mail notification services, call center support and address verification services. HFS will purchase a one-year credit monitoring subscription for each affected Medicaid Provider.

Is there an existing State contract available for the required supplies or services? Yes No
