SFSO, is seeking qualified suppliers (“Proposers”) to provide proposals (“Proposal”) for Medi-Cal enrollment assistance services in the San Francisco County Jails. When applicable and practical, Proposers are encouraged to engage contracting teams that reflect the diversity of the City and include participation of businesses and residents from the City’s most disadvantaged communities including, but not limited to the Bayview/Hunters Point, Chinatown, Mission, South of Market, Tenderloin, Visitacion Valley and Western Addition neighborhoods.
The City has approximately 65 departments, ranging from small to very large. Some departments maintain facilities located outside of the geographic limits of the City. 2. Selection Overview
The City shall award a contract to the Proposer that meets the Minimum Qualifications of this Solicitation whose Proposal receives the highest-ranking score. The Evaluation Panel may include staff from various City departments. Proposals will be evaluated based on the criteria outlined herein. If applicable, a Contract Monitoring Division (CMD) Contract Compliance Officer will assess Proposal compliance with Local Business Enterprise (LBE) requirements and assign a rating bonus to Proposal scores. The City at its sole, absolute discretion, shall have two options to extend the term for a period of two years or for a period of one year each, by mutual agreement in writing. The maximum contract period shall not exceed total of five years.
C. Anticipated Contract Not to Exceed Amount
A contract awarded pursuant to this Solicitation shall have a not to exceed (“NTE”) amount of $364,034 for the initial term. All funding is contingent upon the availability of funds for every fiscal year covered under this RFP, satisfactory program performance, and demonstrated need for the proposed services. This amount is based on City’s estimated spend over the advertised initial contract term. Should City’s actual spend exceed its estimated spend
P-690 (01-26) 2 Sourcing Event 0000012057 for the initial term, City may in its sole discretion increase the contract NTE for the initial term.

GOODS AND SERVICES REQUESTED
A. Goods and/or Services Requested
This Solicitation is being issued by SFSO. SFSO is seeking qualified Proposers to provide Proposals for Medi-Cal enrollment services to persons incarcerated in the San Francisco County Jails.
Since the passage of the Affordable Care Act, SFSO has worked in collaboration with the San Francisco Human Services Agency to support the jail population with applying for Medi- Cal benefits. These efforts were initially directed solely at the small percentage of inmates with a known release date. This changed in 2023 when a California statute mandated all counties implement pre-release Medi-Cal application assistance processes for all eligible individuals as part of the California Advancing and Innovating Medi-Cal Justice Initiative (Cal-Aim JI) which strives to ensure persons who are released from county jails receive timely access to Medi-Cal services.
As the chart below demonstrates, the Jail Health Services team is effective in screening the vast majority of persons booked into the jail for insurance status and approximately 20%- 30% of persons booked each month require application assistance.

I. Scope of Work The Scope of Work is to be used as a general guide and is not intended to be a complete list of all work necessary to complete the project.
Enrollment in health insurance is essential to the reentry process, ensuring timely access to any needed behavioral health treatment in the community. The San Francisco Sheriff’s Office, in collaboration with the San Francisco Human Service Agency (SFHSA), is offering opportunities for health care enrollments to all eligible persons incarcerated within the county jail system.
SFSO seeks to fund a Medi-Cal Enrollment Specialist who will work closely with the Department of Public Health’s Jail Health Services as part of their CalAIM Medi-Cal Assistance team. This position will be responsible for interviews to determine eligibility and, if eligible, the submission of Medi-Cal applications.
Jail Health Services staff are responsible for determining the insurance status of all newly booked individuals and will provide the Enrollment Specialist with a daily list of uninsured persons to interview. The SFSO Medi-Cal Enrollment Specialist will conduct in-person interviews with every individual identified as uninsured and will seek to obtain consent for application assistance. Applications will be completed and submitted same day via BenefitsCal.com and include a cover sheet and User Agency identification to flag them as originating from the San Francisco County Jail. Whenever possible, persons who initially decline enrollment assistance are approached two additional times in the event that they change their mind. In addition, an individual may submit an action request at any time during incarceration to request Medi-Cal application assistance.
The Medi-Cal application assistance team maintains a shared file with SFHSA that tracks all jail-submitted Medi-Cal jail applications. SFHSA updates the application status on a regular basis and the Jail Health Services team updates the patient’s medical record accordingly. The successful proposer will also be expected to meet twice per month with SFHSA, SFSO and Jail Health Services to review the number of applications submitted and to troubleshoot any issues with the submitted applications.
The Enrollment Specialist is also responsible for providing applicants with SFHSA’s contact information as part of their application assistance interview in case the applicant is released before the application is processed.
The following are work tasks assumed necessary for this project:
• Processing healthcare enrollments for county jail inmates.
• Examining, sorting, indexing, and reviewing applications for completeness and accuracy;
• Making routine contacts with San Francisco County Jail inmates to ensure enrollment applications are complete
• Assist in conducting follow up correspondence, as needed,
• Compile information and data necessary for the preparation of various departmental reports.
Staffing
All proposers should have qualified staff with an educational background commensurate with job responsibilities. Staff must also be able to meet SFSO guidelines for jail clearance that includes a fingerprint scan and will be required to attend regularly scheduled program meetings and trainings as well as a mandatory civilian orientation that includes training on the Prison Rape Elimination Act (PREA).
P-690 (01-26) 6 Sourcing Event 0000012057
SFSO is contracting units of service and expects contract workers to be punctual and to make proper notifications when unable to attend work. When a contract employee is going to be late for work, the contract employee shall notify the Jail Health Services Medi-Cal Assistance Team at their work location.
Data Management
The successful proposer shall maintain case notes for every client interaction in its information management system to capture process outcomes such as the number of persons declining assistance and how many times they were approached.
On a monthly basis, program staff shall provide SFSO with the following service measures:
• Number of clients approached
• Number of clients who declined assistance
• Number of Medi-Cal applications submitted

Any attempt to communicate with any party other than the Contract Administrator whose name appears in this Solicitation – including any City official, representative or employee – is strictly prohibited. G. Cybersecurity Risk Assessment
As part of City’s evaluation process, City may engage in Cybersecurity Risk Assessment (CRA). CRA may be performed for each entity manufacturing the product, performing technical functions related to the product’s performance, and/or accessing City’s networks and systems.
Where a prime contractor or reseller plays an active role in each of these activities, CRA may also be required for the prime contractor or reseller.
To conduct a CRA, City may collect as part of this Solicitation process one of the following two reports:

• City’s Cyber Risk Assessment Questionnaire: Proposer’s responses to a City’s Cyber Risk Assessment Questionnaire.
The above reports may be requested at such time City has selected or is considering a potential Proposer. The reports will be evaluated by the soliciting Department and the City’s Department of Technology to identify existing or potential cyber risks to City. Should such risks be identified, City may afford a potential Proposer an opportunity to cure such risk within a period of time deemed reasonable to City. Such remediation and continuing compliance shall be subject to City’s on-going review and audit through industry-standard methodologies, including but not limited to: on-site visits, review of the entities’ cybersecurity program, penetration testing, and/or code reviews.
