About this file
The document is a Service Questionnaire for a Request for Qualifications (RFQ) issued by the Larimer County Department of Human Services, specifically for the Division of Children, Youth, and Family Services. The county is seeking to contract with one or more qualified vendors to provide professional laboratory alcohol and drug substance testing services for their current clients. The RFQ requires prospective vendors to complete a comprehensive application packet that includes certification forms, a detailed narrative addressing experience and qualifications, procedures and policies, collaboration capabilities, and a cost proposal. Vendors must submit their complete application through BidNet, with multiple components including an Application Certification Form, Application Narrative, Debarment Certification, Insurance Attestation, and DORA (Department of Regulatory Agencies) Attestation Agreement.
The RFQ evaluates vendor proposals across four key areas: Experience & Qualifications (30 points), Procedures & Policies (30 points), Collaboration (20 points), and Pricing (20 points). Vendors are expected to provide detailed information about their organizational structure, staff diversity, licensing, professional credentials, program understanding, service delivery approach, client population, and monitoring/evaluation methodologies. The cost proposal must be a fee-for-service rate inclusive of all administrative and overhead costs. Additional requirements include providing proof of insurance, compliance with debarment regulations, and potential DORA registration. The document emphasizes the county's expectations for comprehensive, high-quality service delivery and thorough documentation of program effectiveness.
