About this file

The document is a Service Performance Standards file detailing the contractual performance expectations between a Vendor and the Division of Health Care Financing and Policy (DHCFP). The standards outline comprehensive service level measurements and reporting requirements, with specific performance metrics across five key areas: data and reporting, problem resolution, facility reporting, policies and procedures, and compliance reporting. The document establishes quarterly reporting frequency, annual verification processes, and detailed guidelines for measuring and maintaining service quality.

The performance standards include specific monetary penalties for non-compliance, with aggregate penalties not exceeding 20% of total monthly contract value. Penalties range from $100 per day for missed deadlines, late reports, or non-compliant facilities. DHCFP will collect these penalties by withholding corresponding amounts from the Vendor's payment. The standards require 100% accuracy in all delivered files and reports, with any inaccuracies requiring immediate correction. The Vendor must maintain a single point of contact for resolving service level achievement failures and promptly address and communicate any performance issues.
