SECTION 3 – SCOPE OF WORK

3.1 Background and Purpose

Background:

Traditional care coordination services play a crucial role in ensuring that children, youth, and their families receive comprehensive behavioral health services and support. These services, which include COMAR 10.09.90 Mental Health Case Management: Care Coordination for Children and Youth (also known as Targeted Case Management, or TCM), Targeted Case Management Plus (TCM Plus), Customized Goods and Services (CG&S), and Family Peer Support Services (FPSS), are specifically designed to facilitate access to a wide array of resources but often difficult to navigate/ access by youth in need.

The primary objective of these programs is to preserve family unity and foster the youth's stability within their community environment. By providing individualized support and guidance, care coordination services help navigate complex systems, connect families with appropriate providers, and ensure that services are tailored to meet unique needs. This holistic approach aims to empower families, enhance coping mechanisms, and promote long-term well-being for the youth. The integration of various service components, such as family peer support services, as well as customized goods and services, further strengthens the support network available to families, allowing for a more comprehensive and effective care experience.

Purpose:

The State is issuing this Application for the purpose of identifying an Applicant to establish a community-based, statewide service coordination program focused on ensuring youth and their families are educated about the benefits of TCM, as well as TCM Plus, CG&S and FPSS. The selected Applicant will ensure all youth and families deemed eligible for TCM or TCM Plus services are referred and connected to the appropriate entity for screening and referral to the appropriate Care Coordination Organization. Additionally, Applicant will provide the family peer support and serve as fiscal agent for the administration and disbursement of funds for CG&S. The Applicant will include a clear and concise timeline and a workplan addressing the requirements outlined in this RFA.
