The document is a Sensitive Security Information Acknowledgement Form from the City of Phoenix Aviation Department, designed to protect confidential information related to a contract opportunity. The form requires authorized representatives to acknowledge the sensitive nature of certain documents, agreeing to handle and destroy them in compliance with federal regulations (49 C.F.R. Parts 15 and 1520). The related contract opportunity is for Unemployment Cost Management Services, with a proposed five-year term beginning around March 1, 2026, to be awarded by the City of Phoenix City Council.
The solicitation will be conducted through the City's Procurement Portal, with technical support available through OpenGov e-Procurement Portal. The contract will terminate upon reaching the end of its term or through specific termination provisions. While specific pricing details are not provided in this document, the form emphasizes strict confidentiality requirements and the need for authorized personnel with a "need to know" to handle the associated documents, indicating the sensitive nature of the procurement process.
