About this file

This is a Seller Checklist document issued by the City of Philadelphia Procurement Department for Services, Supplies, and Equipment (SS&E) contracts submitted through the PHLContracts portal. The related contract opportunity involves the procurement of DHS tote bags featuring matte lamination with logos in navy blue with white lettering on the front side only, to be awarded to the lowest responsive and responsible bidder. The micro purchase will be awarded as a whole or by line item, whichever serves the city's best interest. Sellers must quote all items to be eligible for award and should not deliver products until notified by the department. All quotes must be submitted through PHLContracts no later than 10:30 AM on the bid opening date, with no quotes accepted after that time. Quotes submitted by fax or mail will not be accepted.

The checklist requires sellers to complete multiple steps prior to submission, including payment of Annual Bid Security Fee, Bid Processing Fee, and Performance Security Fee when applicable, with payment receipts to be attached to the quote. Sellers must acknowledge the bid and all amendments, ensure Local Business Entity (LBE) certification status is indicated, enter item pricing from the provided pricing sheet, answer all mandatory questions, and accept the terms and conditions without exceptions. Required attachments include an executed Consent & Authorization form, Certificate of Insurance, Bid Bond when applicable, and any MBE/WBE solicitation documentation. Bid Bonds and Certificates of Insurance must be mailed or hand-delivered to the Procurement Department at 1401 JFK Blvd., MSB Suite 170, Philadelphia, PA 19102. The document emphasizes that sellers must maintain the Seller role (not Seller Admin) and use the Summary Tab to review and verify all information before final submission.
