About this file

This is a Seller Checklist document issued by the City of Philadelphia Procurement Department for Services, Supplies, and Equipment (SS&E) procurements through the PHLContracts platform. The checklist applies to a micro purchase for infant and toddler car seats in Pennsylvania, to be awarded as a whole or by line item to the lowest responsive and responsible bidder. Sellers must quote all items to be eligible for award and may not deliver or provide services until notified by the department. All quotes must be submitted through PHLContracts no later than 10:30 AM on the bid opening date, with no quotes accepted after that time. The seller bears responsibility for ensuring timely submission.

The checklist outlines comprehensive submission requirements including payment of Annual Bid Security Fees, Bid Processing Fees, and Performance Security Fees prior to quote submission. Sellers must acknowledge the bid and all amendments, select Local Business Entity (LBE) certification status if applicable, enter item pricing, answer mandatory questions, and add subcontractors to meet any contract participation goals for MBE and WBE participation. Required attachments include executed Consent and Authorization forms, certificates of insurance, bid bonds when applicable, pricing sheets, MBE/WBE solicitation documentation, payment receipts, and the completed Annual Bid Security Application. No exceptions to terms and conditions are permitted for SS&E contracts. Original bid bonds and certificates of insurance must be mailed or hand-delivered to the Procurement Department at 1401 JFK Boulevard, MSB Suite 170, Philadelphia, PA 19102.
