About this file

This document is a Seller Checklist for Small Order Purchases issued by the City of Philadelphia Procurement Department through the PHLContracts platform. The checklist provides step-by-step guidance for vendors submitting quotes for Medical Chart Dividers, a micro purchase opportunity that will be awarded as a whole or by line item to the lowest responsive and responsible bidder. Sellers must quote all items to be eligible for award and should not deliver products or services until notified by the department. The bid is structured as a requirements contract with a term running through June 30, 2027, and includes three optional one-year renewal periods.

The submission process requires vendors to complete multiple tabs within the PHLContracts system, including General, Items, Questions, Terms & Conditions, Attachments, and Summary sections. All vendors must maintain an executed Consent & Authorization form in their vendor profile, and no quote will be considered without this documentation on file. Vendors are also required to answer all mandatory questions, enter unit costs for each listed item, accept the terms and conditions of the bid, and optionally submit an LGBTQ Applicant Opportunity Data Form. The Local Business Entity (LBE) certification question must be answered to proceed, though the LBE Program does not apply to Small Order Purchases. Completion of all required steps and resolution of any validation warnings is necessary before final quote submission through the PHLContracts platform.
