About this file
This document is a Seller Checklist for Small Purchases issued by the City of Philadelphia Procurement Department through the PHLContracts platform for a micro purchase of Dumor Benches and Plaques in Pennsylvania. The checklist outlines the submission requirements for vendors responding to this procurement opportunity. The micro purchase will be awarded as a whole to the lowest responsive and responsible bidder, and sellers must quote all items listed to be eligible for award. No delivery or provision of services should occur until the department provides notification. This procurement will result in a Requirements contract.
The checklist requires sellers to complete multiple steps including confirming seller role status, saving progress after each tab, answering questions regarding Local Business Entity (LBE) certification status, entering unit costs for each listed item, answering all mandatory questions, accepting terms and conditions, and submitting required attachments. Critical submission requirements include providing an executed Consent & Authorization form in the vendor profile, as quotes will not be considered without this documentation on file. Vendors may optionally submit an LGBTQ Applicant Opportunity Data Form and should attach any documents relevant to their quote. All red validation warnings must be addressed before submission through the PHLContracts.phila.gov platform.
