About this file

This is a Seller Checklist document issued by the City of Philadelphia Procurement Department for submission of quotes on small order purchases through the PHLContracts platform. The checklist pertains to a micro purchase for Voting Machine Seals from the City Commissioners Office in Pennsylvania. The procurement will be awarded as a whole or by line item, whichever serves the City's best interest, to the lowest responsive and responsible bidder. Sellers must quote all items listed to be eligible for award consideration. No specific quantities, response deadlines, site visits, or bidder conferences are detailed in this checklist document.

The checklist provides procedural guidance for vendors submitting quotes rather than pricing or contract terms information. Sellers must complete several mandatory steps including confirming seller role status, answering whether their business is certified as a Local Business Entity, entering unit costs for each item, answering all mandatory questions, accepting terms and conditions, and submitting a Consent & Authorization form. An optional LGBTQ Applicant Opportunity Data Form may be attached. The document emphasizes that vendors should not deliver goods or provide services until formally notified by the department, and that all red validation warnings must be addressed before quote submission through www.PHLContracts.phila.gov.
