About this file

This is a Seller Checklist document issued by the City of Philadelphia Procurement Department for small order purchases, specifically related to a corrugated boxes procurement opportunity (CCOM Corrugated Boxes) in Pennsylvania. The checklist provides step-by-step guidance for vendors submitting quotes through the PHLContracts.phila.gov platform. Sellers must complete multiple tabs including General, Items, Questions, Terms & Conditions, Attachments, and Summary sections. The procurement will be awarded as a whole or by line item, whichever serves the City's best interest, to the lowest responsive and responsible bidder. Sellers are required to quote all items to be eligible for award and must not deliver services or products until receiving formal notification from the department.

Submission requirements include selecting appropriate seller role credentials, confirming Local Business Entity (LBE) certification status, entering unit costs for each item, answering all mandatory questions, and accepting terms and conditions. Vendors must submit a valid executed Consent & Authorization form with their quote or have one on file in their vendor profile; quotes lacking this documentation will not be considered. Sellers may optionally attach an LGBTQ Applicant Opportunity Data Form and any supporting documents related to their quote. All attachment requirements and validation warnings must be addressed before final submission through the PHLContracts platform.
