About this file

This is a Self-Performance Requirements document for the Illinois Department of Human Services (DHS) related to a rebid project to replace two underground water distribution valve systems at the Ann Kiley Center located at 1401 W. Dugdale Road in Waukegan, Illinois, scheduled for fiscal year 2027. The document establishes requirements regarding the extent to which the selected contractor must perform work directly rather than through subcontractors, ensuring accountability and quality control for this critical infrastructure replacement project at the state facility.

The self-performance requirements specify the minimum percentage or scope of work that must be completed by the prime contractor's own workforce and resources. These provisions are designed to maintain contractor responsibility, prevent unnecessary subcontracting, and ensure adequate oversight of the valve system replacement work. The document serves as a binding component of the contract terms and conditions that all bidders must acknowledge and comply with upon award.

MANDATORY SELF-PERFORMANCE

· Minimum Performance: The Prime Vendor shall perform on the site, and with its own organization, work equivalent to at least 51% of the total amount of work to be performed under this Contract.

· Definition of "Own Organization": The term "its own organization" refers strictly to personnel employed and compensated directly by the prime vendor, as well as equipment owned or leased by the prime vendor, whether operated or non-operated. Self-performance must be executed exclusively by permanent, full-time W-2 employees who have actively performed services for and been paid verifiable wages by the prime vendor for a minimum of 30 consecutive days prior to bid submission. The use of staff augmentation, 1099 independent contractors, and agency or temporary labor shall not qualify toward self-performance requirements.

· Evidence of Own Organization Performance: The percentage of prime vendor’s performance may be verified via the Illinois Department of Labor’s Certified Payroll Report as well as visitor logs from the facility.

· The prime vendor’s employees shall carry identification, supplied by the prime vendor, with the business name, employee name and photo, at all times.

· Non-Compliance: Failure of the prime vendor to meet the self-performance percentage specified herein at the time of bid submission, or a post-award deviation without prior written approval from the State, shall constitute a material breach of contract and shall be grounds for immediate termination for cause.
