About this file

This document is a Schedule B - Pricing Request for Proposal (RFP) for the provision of security paper to the Michigan Department of Health and Human Services (MDHHS) Division for Vital Records & Health Statistics. The RFP is seeking proposals for two configurations of security paper in both 8 1/2" x 11" and 8 1/2" x 14" sizes, with estimated annual quantities ranging from 60,000 to 200,000 sheets. The paper must include specific design elements such as raised seals and logos. The contract term is for 2 years. Bidders must provide a pricing schedule for the proposed contract activities, including any one-time or set-up charges, fees, and potential costs. Bidders are encouraged to offer quick payment terms. Orders will be placed through a web-based order system, with local registrar offices ordering and receiving shipments directly rather than through the State of Michigan.

The pricing schedule includes per case pricing for the different paper configurations, with the annual and 2-year contract values to be calculated. Imprint charges for local logos and custom local seals are also provided. Bidders must certify that prices were arrived at independently. The security paper will be used by state and local registrar offices to provide certified copies of vital records to individuals for various purposes such as obtaining passports, driver's licenses, and accessing government benefits.

1. The Contractor must provide a pricing schedule for the proposed Contract Activities using the table below. The pricing schedule should be submitted in a modifiable format (e.g., Microsoft Word or Excel); however, you may also submit an additional pricing schedule in a non-modifiable format (e.g., PDF). Failure to complete the pricing schedule as requested may result in disqualification of your proposal.

2. Price proposals must include all costs, including but not limited to, any one-time or set-up charges, fees, and potential costs that Contractor may charge the State (e.g., shipping and handling, per piece pricing, and palletizing).

3. The Contractor is encouraged to offer quick payment terms. The number of days must not include processing time for payment to be received by the Contractor's financial institution.

Quick payment terms: % discount off invoice if paid within days after receipt of invoice.

4. By submitting its proposal, the Contractor certifies that the prices were arrived at independently, and without consultation, communication, or agreement with any other Contractor.

Sheets are to be packed 2,000 sheets per carton. Beginning and ending numbers must be listed on each carton. The contents of the carton are to be plastic wrapped in quantities of 500 sheets. Cartons must be sealed with printed security tape to deter and reveal tampering with sealed cartons. Only full cartons will be shipped.

Paper must be produced in two different sizes with two different seal and logo configurations:

State Registrar configuration:

Both 8 ½ “ x 11” and 8 ½” x 14” page sizes with two State raised seals over the State Coat of Arms and State Vital Records logo. This configuration may be ordered by both the State office and Local Clerk offices.

Local Registrar configuration:

Both 8 ½ “ x 11” and 8 ½” x 14” page sizes with one State raised seal over the State Coat of Arms and one local logo in place of State Vital Records logo. With this configuration each local would have the option for a second raised seal over the local jurisdiction logo.

Please Note: The above Security Paper prices should be based on “production” order quantities and NOT “release” order quantities. For example, if the State places a purchase order for 75 cartons, the price per carton will reflect the “production” quantity price. However, the State, city, or county may “release” for shipment in whatever quantities are required (even small shipments) without impacting the carton price. Bidder needs a web- based order system. These orders should be made on the web-based order entry system that is set up for the State and all Cities and Counties in Michigan to place an order. When a City or County places an order, it should be approved by the State office before processing by vendor. Orders placed by local offices should be shipped and invoiced directly to each county or city per “release” and not to the State of Michigan.
