About this file

This file is a sector representatives table template for the Drug-Free Communities (DFC) Support Program grant application. Applicants must complete all boxes of the table by identifying individual sector representatives from their community coalition, providing their organization affiliations, and explaining their qualifications to represent each sector. The template requires representation from twelve distinct sectors: youth (with specified age), parent, business, media, school, youth-serving organization, religious or fraternal organization, law enforcement (non-retired), civic/volunteer groups, healthcare professional or organization, state/local/tribal government agency with substance abuse expertise, and other organizations involved in substance abuse reduction. The selection rationale for each representative must demonstrate alignment with sector representative definitions outlined in the program's Notice of Funding Opportunity (NOFO).

The DFC Support Program, established under the Drug-Free Communities Act of 1997 and administered by the Department of Health and Human Services Centers for Disease Control and Prevention (CDC), provides grants to community coalitions to prevent and reduce substance use among youth aged 18 and younger. The FY 2026 competition is open to both new and continuing applicants. Coalitions must demonstrate at least six months of prior collaboration on substance abuse reduction, meet composition requirements including this sector representation standard, provide 100-150 percent cash or in-kind matching funds, and commit to program evaluation. Typical awards range from $250,000 to $500,000 annually for performance periods of one to five years, with continuation awards available based on satisfactory progress.
