About this file
This document is a template for identifying local area concerns within a half-mile radius of a proposed site, likely for a Request for Proposal (RFP). The form requires the offeror to comprehensively document nearby facilities that might raise public concern, specifically including schools, day-care centers, historical landmarks, and other residential facilities. The template provides three columns for contractors to record: (1) Business Name & Address, (2) Distance From Proposed Site, and (3) Offeror's Narrative Concerning Possible Opposition Near Proposed Site. The instructions explicitly state that there is no limit to the number of entries that can be made, emphasizing the need for thorough documentation of potential local community impacts or concerns surrounding the proposed site location.
