About this file
This document is a section from a state and local government contract opportunity related to the Connor Springs SD Main Extension project. It outlines the certification, license, and test requirements for materials and personnel used in the construction of this project. The key requirements include:
- Certifications for fusion procedures, persons performing fusion, concrete, employee alcohol misuse and anti-drug plans, and operator qualifications. 2) The contractor must hold a current utility contractor license issued by the State of Georgia Construction Industry Licensing Board, with a certified utility manager or foreman overseeing the work. 3) Certifications from manufacturers are required for all valves and fittings used in the project. The engineer may require additional testing as needed, with the costs borne by the owner.
The document also includes an Appendix A that lists the specific covered tasks the contractor's personnel must be qualified to perform, such as valve operation, pipe joining, welding, and meter/regulator installation. This demonstrates the extensive technical expertise and certifications required for personnel working on this state and local government infrastructure project.

Each procedure shall be qualified to produce fusion connections with suitable mechanical properties and soundness meeting the requirements of the Pipeline Safety Regulations, Code of Federal Regulations, Title 49, Latest Edition, Part 192.283 Plastic pipe: qualifying joining procedures. The procedure qualification test report shall state that the fusion procedure was tested in accordance with this Part and the test results were found to be satisfactory.
Two (2) copies of the written fusion procedure to be implemented on this project meeting the requirements of this Paragraph and two (2) copies of the procedure qualification test report shall be furnished to the Engineer.
2. Certification of Persons Performing Fusion: All fusion shall be performed by persons qualified in the fusion of polyethylene pipe and tubing in accordance with the procedures required above. Each person engaged in fusion shall be qualified to produce fusion joints with suitable mechanical properties and soundness meeting the requirements of the Pipeline Safety Regulations, Code of Federal Regulations, Title 49, Latest Edition, Part
192.285 Plastic pipe: qualifying persons to make joints. The qualification test report shall state that the person performing the fusion of polyethylene pipe and tubing has demonstrated their ability in the fusion procedure in the presence of the polyethylene manufacturer by making a specimen joint according to the fusion procedure above and that this joint was found to be satisfactory in accordance with this Part. The qualification test report shall be issued by the polyethylene manufacturer in accordance with the Pipeline Safety Regulations, Code of Federal Regulations, Title 49, Latest Edition, Part 192.287 Plastic pipe: inspection of joints. A copy of the certified test report shall be furnished to the Engineer.
