The RFQ seeks qualified vendors to provide structural vehicle modifications, installation of mobility equipment and adaptive driving devices, high-tech driving systems, equipment adjustments, and repair services for vocational rehabilitation clients throughout Nevada. Current service agreement holders Ability Center and Mobile Works are specifically invited to submit proposals. Vendors must submit a completed Vehicle Modification and Conversion List of Services form, copies of applicable licenses and certifications, a detailed description of proposed services, geographical service locations, employee and technician information, and a minimum of three references.

Vendors must charge parts and materials at no more than 20% markup above cost, with the state reserving the right to verify invoices. All vendors must be accredited for the services they perform and employ certified technicians for equipment installation and servicing. Prior to work, vendors must assess vehicle structural integrity and, for vehicles three years or older or with 50,000+ miles, conduct documented inspections certifying good condition. All completed modifications must meet Federal Motor Vehicle Safety Standards and factory specifications, with vendors required to sign an Installer Affidavit of Completion form and submit detailed invoices with supporting documentation. Payment is contingent upon receipt of all required documentation. QAP accreditation by the National Mobility Equipment Dealers Association is preferred but not required, and vendors may optionally offer vehicle transportation services.

Commodity Codes: 470-80, 071-04, 071-92

Overview: The Department of Employment, Training and Rehabilitation, Bureau of Vocational Rehabilitation is seeking proposals from qualified vendors to provide services for modifying and converting new and used vehicles for vocational rehabilitation clients. Modifications/conversions include structural vehicle modifications; installation of mobility equipment and devices including customized and advanced adaptive driving equipment and controls; installation of high-tech driving systems; adjustments to fit required equipment to specific needs and equipment repair services as needed.

Scope of Work:

Services will include, but are not limited to, providing modification and conversion of new and used vehicles for Vocational Rehabilitation clients as follows: structural vehicles modifications; installation of mobility equipment and devices including customized and advanced adaptive driving equipment and controls; installation of high tech driving systems; provide adjustments to fit required equipment to specific needs; and provide equipment repair services as needed.

Vendors must provide a detailed description of their proposed services, which may include those listed on the attached Vehicle Modification and Conversion List of Services. Vendors must be accredited for all of the types of work they perform and must have certified technicians in the equipment they sell, install and service. Vendors must return a completed Vehicle Modification and Conversion List of Services form and submit copies of all applicable licenses and/or certifications. Vendors do not need to be qualified to provide all listed services to be considered eligible for award.

Vocational Rehabilitation clients are located throughout the State of Nevada. Vendors must provide a detailed list of all their Geographical Service Location(s) where the proposed services will be performed. Vendors do not need to provide statewide service locations to be considered eligible for award.

Prior to performing requested services, Vendor must:

· Assess whether the vehicle can structurally support the requested services. Already modified equipment must be included in this assessment.

· If the vehicle is three (3) years old or more; or has 50,000 miles or more, inspect the vehicle to determine its condition and including the reliability of the engine and/or other appropriate systems such as the power train, brakes, electrical system, body and safety features. The results of this inspection must be documented and the Vendor must provide a written certificate that the items have been inspected, are in good condition and the vehicle warrants requested services.

· DETR reserves the right to have the vehicle inspection performed by an independent vendor. This does not remove the vehicle modification vendor’s responsibility to notify DETR staff of any noticeable repairs required or reasons why a modification may not be warranted for the vehicle.

· Due to the nature and extent of work varying for each vehicle, Vendor must provide a written cost estimate/quote based on each vehicle modification and conversion requirements, detailing labor and parts/material costs for prior approval. Cost estimate/quote shall be submitted to the Rehabilitation Administrator or designee.

· Parts/Materials shall be invoiced at no more than 20% markup above Vendor’s cost. The state reserves the right to request copies of the Vendor’s parts and material invoices to verify.

Upon completion of requested services, Vendor must:

· Any modifications, installations, adjustments and repairs must meet factory specifications and conform to the Federal Motor Vehicle Safety Standards (FMVSS).

· If it is anticipated that the vehicle will not conform with the FMVSS after requested services are completed, Vendor must have an exemption from the “make inoperative” prohibition mandated through the National Highway Traffic Safety Administration (NHTSA) (49 CFR Part 595).

· Sign and date an Installer Affidavit of Completion of Vehicle Modification VM-5 Form (to be provided by the Rehabilitation Division) certifying that the work has been completed in compliance with the prescription and/or specifications and written warranties have been provided to the participant.

· Submit detailed invoice along with all supporting documentation and the VM-5 Form upon completion of each service, modification and/or conversion. Payment will not be made to the Vendor without all required documentation.
