About this file
This Scope of Work document outlines laundry services to be provided to Grand Mesa Youth Services by a contractor under a Colorado Department of Human Services, Division of Youth Services (DYS) contract. The contractor shall deliver clean linen and other items while collecting soiled linen according to a delivery schedule that may be modified by mutual agreement between the State and Contractor. Orders may be placed via fax, email, web-based systems, or phone. All clean linen must be wrapped, marked, and stored in carts with impervious plastic bags, while soiled linen must be transported in separate closed carts to prevent cross-contamination. The contractor is responsible for maintaining sufficient inventory at the facility, providing a point of contact for discrepancies with a four-hour emergency delivery response time, and submitting invoices specifying quantities of each item delivered. The contractor must adhere to all applicable DYS policies, including those related to facility access, background checks, and employee-juvenile relationships. The contract term spans from November 2026 through November 2030, with pricing to be negotiated annually.
Payment shall be made monthly based on expenditure statements submitted on State-prescribed forms, not to exceed the current fiscal year contract maximum. The contractor must maintain a double-entry accounting system compliant with Generally Accepted Accounting Principles (GAAP) with adequate timekeeping and cost allocation systems. If the contractor receives $400,000 or more in state funds annually, an independent audit of financial statements is required within six months of the contractor's fiscal year end, with any audit deficiencies requiring a corrective action plan within four months. The State reserves the right to make service requirement changes with 30 days' notice, though unit pricing shall not be affected during the initial contract term; changes to pricing after the initial period require negotiation and written approval no less than 90 days prior to contract term end. Failure to comply with contract requirements, including financial reporting and audit submission, may result in contract termination or payment withholding. The contractor is responsible for all ordinary inventory losses and must disclose subcontracting arrangements. Specific pricing rates were not populated in the provided document excerpt.
