About this file

This document outlines the Scope of Services for a State Burial Services contract between Shapiro Center and a Funeral Director vendor. The vendor will be responsible for providing full burial services, including the removal of remains, embalming, casket and concrete vault, transportation, and coordination with Shapiro Center's Records Office for necessary documentation. The vendor must comply with all applicable local, state, and federal codes, standards, laws, and regulations. Shapiro Center's Chief Engineer/designee will serve as the contract monitor, and the vendor must complete billing per the contract specifications within 30 days.

The contract does not specify any pricing terms, set-asides, incumbent information, or funding sources. However, it does state that the vendor will be responsible for providing a cemetery lot within a 25-mile radius of Shapiro Center and for the installation of a grave marker for each deceased individual covered under the contract. No vendor travel expenses will be paid by Shapiro Center unless an unusual situation occurs, which would require review and approval by the Fiscal Services Director.
