About this file

This is a Schedule C Bidder Response form for Request for Proposal No. 260000001094 issued by the Michigan Department of Health and Human Services (MDHHS) for the WIC Infant Formula Rebate Program. The RFP solicits proposals from infant formula manufacturers to participate in a rebate program serving the Women, Infants, and Children (WIC) program in Michigan. Bidders must respond to compliance requirements across multiple schedule sections covering product specifications, formula supply requirements and procedures, quality assurance programs, transition schedules, data retention functionality, timeframes, payment methods, work plans, and rebate invoice billing procedures. All response items are evaluated on a pass/fail basis, with proposals receiving any fail determination automatically disqualified from consideration for award. Bidders must affirm compliance with Standard Contract Terms and submit their nationally published wholesale price lists effective on the bid opening date for each type and form of infant formula in their product line.

Bidders must certify registration with the Secretary of Health and Human Services under the Federal Food, Drug and Cosmetic Act and confirm that all formulas comply with applicable federal regulations (21 USC 301 et seq and CFR 246.10(g)). The evaluation process occurs in two steps: Step 1 assesses pass/fail requirements across all specified sections, and Step 2 evaluates pricing as outlined in Schedule B. Bidders have the opportunity to propose modifications to specified requirements; modifications will result in a pass determination only if the State determines them acceptable at its sole discretion. The file does not specify response deadlines, award dates, or contract term duration, as these details are contained in other sections of the RFP documentation.
