About this file
This is a bid pricing sheet template issued by the Michigan Department of Health and Human Services (MDHHS) for the WIC Infant Formula Rebate Program (RFP 26-1046), designed to collect competitive bids from manufacturers for primary contract infant formula. The solicitation requires bidders to submit pricing for three physical forms of infant formula—powdered, liquid concentrate, and ready-to-feed—across three infant age categories (0-3 months, 4-5 months, and 6-11 months). Bids must account for both fully formula-fed and partially breastfed infants, with estimated monthly issuance quantities ranging from approximately 28,751 ounces for liquid concentrate to 21,075,474 ounces for powdered formula. The bid sheet requires manufacturers to provide data to at least three decimal places and comply with federal requirements outlined in 7 C.F.R. 246.10(e)(9)(Table 1) and 246.16a(c)(2, 4-7).
Bidders must submit the lowest wholesale full truckload price per unit and a rebate bid per unit for each product category, from which the net price per unit and percent rebate will be calculated. The template includes estimated average infant populations based on six months of recent program data, excluding exclusively breastfed infants and those issued exempt formula, to establish standardized unit quantities for cost calculations. Bidders must certify compliance with the Food, Drug and Cosmetic Act, meet all physical form requirements suitable for healthy full-term infants, and commit to supplying one hundred percent of the WIC Program's needs across all geographic areas served by Michigan. No assurances are provided regarding actual quantities, types, or physical forms that will be issued under any resulting contract.
