About this file

This Schedule C bidder response document is part of Request for Proposal No. 260000001094 issued by the Michigan Department of Health and Human Services (MDHHS) for the WIC Infant Formula Rebate Program. The RFP seeks qualified bidders to supply infant formula products and manage rebate calculations for the Women, Infants, and Children (WIC) program. Bidders must demonstrate compliance across multiple specification sections including product specifications, formula supply requirements and procedures, quality assurance programs, data retention and retrieval functionality, transition planning, contractor representation, and organizational structure. The evaluation process follows a Pass/Fail determination methodology, with proposals receiving a FAIL rating in any section being ineligible for award consideration. Bidders must submit responses indicating full compliance, non-compliance, or compliance with modifications for each RFP section, with modifications subject to State discretion for acceptance.

Bidders are required to submit their nationally published wholesale price lists effective on the bid opening date for each type and form of infant formula in their product line, with pricing evaluation occurring as Step 2 of the evaluation process following completion of all Pass/Fail requirements. All bidders must certify registration with the Secretary of Health and Human Services under the Federal Food, Drug and Cosmetic Act and confirm formula compliance with federal regulations. The RFP mandates submission of supporting documentation including work plans, risk management vendor questionnaires, and affirmation of agreement to Standard Contract Terms. Bidders must also provide contractor representative information, organizational charts, and demonstrate capability for rebate invoice billing, payment procedures, error management, and quality assurance protocols throughout the contract term.
