About this file

This is a pricing schedule for a Request for Proposal (RFP) issued by the State of Michigan, Department of Juvenile Justice and Human Services (DJJHV), for the procurement of medical apparel and accessories under RFP No. 511 180000003418. The contract is designed to serve approximately 157 eligible employees during the 2019 fiscal year (October through September 30). The proposal requires contractors to provide pricing for scheduled ordering events where employees will have the opportunity to select from available products across two designated dates. The total employee allocation is $12,560, calculated at $80 per employee (157 eligible employees × $80). DJJHV will not be held responsible for actual attendance numbers at the scheduled events, and vendors are encouraged to advertise available products and dates within the department to maximize employee participation.

Contractors must submit comprehensive pricing schedules in a modifiable format (Microsoft Word or Excel), with all-inclusive costs including setup charges, shipping and handling, per-piece pricing, and palletizing fees. Proposals that fail to complete the pricing schedule as specified may be disqualified. Contractors are encouraged to offer quick payment discount terms and must certify that prices were developed independently without consultation with other contractors. The base unit price of $80 per employee totaling $12,560 must be entered both in the detailed pricing table and in the RFP's commodity unit price section. The employee population may fluctuate due to retirements, new hires, and position changes.
