About this file

This is a Schedule B Pricing document for Michigan's Request for Proposal No. 260000002410, which solicits administrative services for Behavioral Health and Substance Abuse Benefits under the State Health Plan PPO. Contractors are required to submit pricing proposals for a three-year contract period commencing January 1, 2028, and ending December 31, 2030. The pricing schedule must be completed in modifiable format (Microsoft Word or Excel), with only light blue-shaded cells populated by bidders. Contractors must include all costs in their proposals, including one-time setup charges and any fees they may charge the State. The contract covers projected membership of approximately 97,300 active members and 19,072 retirees in 2028, increasing to 103,226 active members and 20,233 retirees by 2030.

Pricing proposals must be submitted on a Per Contract Per Month (PCPM) basis for Administrative Service Only (ASO) fees, inclusive of Applied Behavior Analysis (ABA) case management fees, along with claims cost projections utilizing an 8 percent trend factor. Contractors must reprrice historical claims data from Schedules M1, M2, M3, and M4 (representing active and retiree populations for fiscal years 2024 and 2025) and provide pass-through pricing to the plan sponsor without charging amounts exceeding provider payments. Additional pricing information required includes telehealth reimbursement rates segmented by provider level, out-of-network reimbursement methodologies, shared-savings programs, and average reimbursement levels for both national and Michigan-specific employer book of business by CPT code and provider type. Bidders are encouraged to offer quick payment discount terms. Price proposals must certify independent pricing development without consultation or agreement with other contractors. The State of Michigan does not guarantee minimum or maximum service volumes, claims, or conversions under the contract.
