About this file

This is a Summary of Benefits and Coverage (SBC) document for the City of Greenville, Greenville Utilities Commission, and Greenville Housing Authority's Choice Fund Open Access Plus HSA health plan effective January 1, 2026 through December 31, 2026. The plan is administered by Cigna Healthcare and provides medical, pharmacy, and behavioral health coverage for individual and family enrollees. The document outlines comprehensive coverage details including deductibles, copayments, coinsurance percentages, and out-of-pocket maximums for various healthcare services ranging from primary care visits and preventive care to emergency services, hospitalization, maternity care, mental health services, and rehabilitation. In-network deductibles are $1,700 per individual or $3,400 per family, while out-of-network deductibles are $3,200 per individual or $6,400 per family. The employer contributes up to $500 per individual or $1,000 per family to HSA accounts. Most covered services require 20 percent coinsurance for in-network providers and 40 percent for out-of-network providers after the deductible is met.

The plan includes an in-network out-of-pocket maximum of $3,000 per individual or $6,000 per family, and an out-of-network maximum of $6,000 per individual or $12,000 per family. Preventive care and immunizations are covered at no cost without deductible application. The plan excludes coverage for dental care (adult and children), routine eye care, infertility treatment, long-term care, and weight loss programs, among other services. Covered services include bariatric surgery (in-network only), chiropractic care, and hearing aids with specified limitations. The document provides three coverage examples demonstrating potential patient costs under different scenarios: maternity care totaling $3,020 in patient responsibility, diabetes management totaling $2,440, and emergency fracture treatment totaling $1,900. The plan meets Minimum Essential Coverage and Minimum Value Standards requirements. Language assistance services are available in multiple languages including Spanish, Chinese, Vietnamese, Korean, Arabic, and French Creole.

Summary of Benefits and Coverage: What this Plan Covers & What You Pay for Covered Services Coverage Period: 01/01/2026 - 12/31/2026 City of Greenville, Greenville Utilities Commission and Greenville Housing Authority: Choice Fund Open Access Plus HSA

Coverage for: Individual/Individual + Family | Plan Type: OAP

The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately. This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, go online at www.cigna.com/sp. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at https://www.healthcare.gov/sbc-glossary or call 1-800-Cigna24 to request a copy.

Important Questions Answers Why This Matters:

What is the overall deductible?

For in-network providers: $1,700/individual - employee only or $3,400/family maximum For out-of-network providers: $3,200/individual - employee only or $6,400/family maximum Combined medical/behavioral and pharmacy deductible Deductible per individual applies when the employee is the only individual covered under the plan.

Amount your employer contributes to your account: Up to $500/individual or $1,000/family

Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the policy, the overall family deductible must be met before the plan begins to pay.

Are there services covered before you meet your deductible?

Yes. In-network preventive care & immunizations, in-network preventive drugs.

This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost-sharing and before you meet your deductible. See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits/.

Are there other deductibles for specific services? No. You don't have to meet deductibles for specific services.

What is the out-of-pocket limit for this plan?

For in-network providers: $3,000/individual - employee only or $6,000/family maximum For out-of-network providers: $6,000/individual - employee only or $12,000/family maximum Combined medical/behavioral and pharmacy out-of-pocket limit

The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, the overall family out-of-pocket limit must be met.

What is not included in the out-of-pocket limit?

Penalties for failure to obtain pre-authorization for services, premiums, balance-billing charges, and health care this plan doesn’t cover.

Even though you pay these expenses, they don't count toward the out-of-pocket limit.

Important Questions Answers Why This Matters:

Will you pay less if you use a network provider?

Yes. See www.cigna.com or call 1-800-Cigna24 for a list of network providers.

This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.

Do you need a referral to see a specialist? No. You can see the specialist you choose without a referral.

All copayment and coinsurance costs shown in this chart are after your deductible has been met, if a deductible applies.
