SAMPLE REPORT GUIDELINES

1. Provide name of program and program dates

2. Provide a description of and evaluate each company/site visit.

• Evaluations should consist of a short narrative of what was positive and/or negative about the visit and why the visit was given the particular rate.

• Each site evaluation should contain the location (city), company name and company representatives.

• Do not list the topics covered UNLESS they vary from the topics listed in the schedule. For example, if the schedule noted that the company was asked to talk about HR, but they spent an hour answering the group’s questions about taxation, please note that. We want to know what topics were covered.

• We want to determine whether (if the opportunity arises) a site visit should definitely be repeated, repeated with suggested improvements or not repeated.

Please note: ratings should be based on the delegation’s evaluation of the visit.
