QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

Building 101 Turnstiles

May 8, 2015

1. Objective

The purpose of this Quality Assurance Surveillance Plan (QASP) is to document the procedures, guidelines and evaluation criteria the Government will use to ensure the Contractor meets the performance requirements of the statement of work (SOW) and specifically identified in the Performance Requirement Summary. It details (a) the performance standards that will be used to determine whether the performance requirements are being met, (b) the method of surveillance, and (c) how performance results will be documented

1. Performance Standards:

The CO and COR will monitor the Contractor to determine whether the Performance Standards set forth in the Task Order have been attained. Performance Standards for all tasks are specified in the Performance Requirements Summary. The Contractor is responsible for making required changes to ensure performance is managed effectively.

The Contractor is responsible for adequately demonstrating that it has met or exceeded performance standards. NIST will use a number of surveillance methods to validate performance information provided by the Contractor. These methods may include random checks, observations, inspections, and the review of records and files that the Contractor is required to maintain and delivery under the Task Order.

A) COR will review periodically (at least one a week) and unannounced listing of construction contractor personnel requesting access to NIST and granted access within the number of days permitted in the SOW. Success is 95% compliance

B) Review the CQM daily inspection reports at least twice a week for compliance against the task order SOW. Success is 95% compliance

C) Unannounced several times during the task order review the CQM activities while the construction contractor is constructing the turnstiles for compliance against the task order SOW for items not noted within the QASP. Success is 95% compliance

D) At least twice a week, review the submittal tracking system established by the CQM Success is 90% compliance

E) At least once a month, review the CQM record keeping as specified in the task order.

Success is 90% compliance

F) Upon issuance of a CPM schedule chart review by the CQM review no less than 4 days after issuance for compliance with task order performance measures Success is 90% compliance

G) At least twice a week, review all photographs being taken.

Success is 90% compliance

H) Complete turnstile commissioning to ensure that all units operate per the manufacturer’s instruction. In addition access control features and fire alarm interoperability function per the construction drawings.

Success is 100% compliance

3. Quality Assurance Reporting

The CO and COTR will develop a Quality Assurance Findings Report at least once every 60 days. These reports will be used to develop annual Performance Evaluation Reports as a whole. The CO and COR will review all findings in the Quality Assurance Findings Report with the Contractor and provide feedback on its performance under each element identified in the Performance Requirements Summary.

Report will address the performance standards in paragraph 2.
