About this file

This is a Supportive Services Agreement template between a participant and provider, related to Pierce County, Washington's Continuum of Care program under the U.S. Department of Housing and Urban Development (HUD). The agreement establishes a framework for creating a Supportive Services Plan designed to help participants achieve self-sufficiency through goal-oriented interventions. The plan encompasses case management and may include services such as employability training, substance use treatment, financial literacy, education, behavioral healthcare, community service, and household management. The Participant and Provider commit to reviewing and adjusting the plan at least monthly to track progress and ensure continued relevance. The document includes signature lines for both parties to formalize the agreement and acknowledge their mutual commitment to the plan's implementation.

The agreement incorporates accountability measures, establishing that failure to comply with the agreement may result in termination of assistance.
