About this file
This is a Sample Insurance Certificate associated with the City of Pembroke Pines' competitive sealed bid solicitation for epoxy replacement of the Great Hall flooring at Charles F. Dodge City Hall, located at 601 City Center Way, Pembroke Pines, Florida, in Broward County. The project involves complete epoxy flooring installation and renovation services using the Sherwin-Williams Resuflor Deco Flake BC system with a nominal thickness of 20-30 mils, including demolition, surface preparation, moisture testing, primer application, topcoating, sealant application, and slip-resistant finish completion. The contractor must submit comprehensive documentation including product data, installer certificates, material certificates, maintenance data, color and texture samples, and a product schedule prior to installation. A mandatory pre-bid meeting is scheduled for March 3, 2026 at 10:00 a.m., with questions due by March 17, 2026 at 6:00 p.m. and final answers issued by March 23, 2026. Electronic bid submission is required by 2:00 p.m. on March 31, 2026, with public bid opening at 2:30 p.m. the same day. The City Commission will recommend award on May 6, 2026, with Notice to Proceed issued by May 20, 2026. Work must be performed between July 30, 2026 and August 30, 2026, limited to weekends and Thursday evenings after 5:00 p.m. until Sunday at 11:00 p.m., and must be completed within 25 calendar days from issuance of the Notice to Proceed.
The estimated project cost is $150,000, with a proposal security requirement of $10,000 or 5 percent of the total bid amount, whichever is less. The contractor must provide general liability insurance with limits of $1,000,000 per occurrence and $2,000,000 aggregate, workers' compensation coverage, auto liability insurance, and $2,000,000 umbrella/excess liability coverage, with the City named as additional insured. Two separate payment and performance bonds, each equal to 100 percent of the contract price, are required. Contractors must meet minimum qualifications of five years of experience and provide both a seven-year workmanship labor warranty and a seven-year product warranty from the manufacturer. The project applies federal Davis-Bacon prevailing wage requirements and HUD Section 3 provisions, requiring contractors to ensure that at least 25 percent of total project hours are worked by Section 3 workers and 5 percent by Targeted Section 3 workers. Liquidated damages are assessed at $964 per day for failure to meet start or completion dates. The City will waive all City-related permit and license fees, covering state or county fees through an Owner's Contingency allowance.
