About this file

This is a Sample Evaluation Criteria and Rating Plan document for the City of New Haven Department of Health's Request for Proposals to develop a Five-Year Workforce Development Plan. The RFP solicits proposals from qualified consultants to create a comprehensive workforce development plan compliant with Public Health Accreditation Board (PHAB) Accreditation Standards and Measures (Version 2022 or 2026) while building upon the existing 2022-2025 plan. Key deliverables include a project workplan, workforce assessment summary, draft and final Five-Year Workforce Development Plans, a PHAB Workforce Standards crosswalk, and an executive summary. The question submission deadline is March 6, 2026, at 5:00 PM, with proposals due March 17, 2026, at 11:00 AM, submitted electronically through the City's eProcurement Portal. The contract term is one year with no optional extension periods. Proposals must be formatted in 12-point Times New Roman, single-spaced, with a maximum of 10 pages including budget and cost narrative.

Evaluation will be conducted using three equally weighted criteria (25 points each): understanding of NHV Health's needs and PHAB requirements; comprehensive work plan addressing all scope areas; and relevant experience and qualifications. Reviewers will assess proposals using one of three rating scales—adjective-based (Outstanding, Good, Adequate, Fair, Poor, Unacceptable), color-based (Blue, Green, Yellow, Red), or numerical (10, 8, 5, 2, 0)—based on identified strengths and weaknesses relative to stated criteria. The total funding available does not exceed $20,000, with all costs required to be reasonable, necessary, and directly related to the scope of work. Proposers must provide a detailed, all-inclusive budget with no additional compensation beyond the executed contract amount. The contractor will be classified as an independent contractor receiving no fringe benefits, and compensation constitutes full payment for all costs including salaries, consultant fees, materials, supplies, printing, meetings, travel, postage, and clerical expenses. The City reserves the right to waive informalities, reject any or all proposals, and select the proposal deemed most advantageous to the City's interests.

A weakness is an aspect of a Response that, when compared to the stated evaluation criterion, appears to negatively affect the probability of successful mission accomplishment of the potential financial assistance agreement.

After completing individual reviews, the Review Leader should schedule a consensus review meeting and coordinate the development of the Consensus Strengths and Weaknesses and Consensus Scores.

Rating Scale Sample A:

Only the following adjectives may be assigned: Outstanding, Good, Adequate, Fair and Poor. The scoring of each criterion must be based on the strengths and weaknesses of the Response. To assist in assigning an appropriate score, the following can be used as a guideline:

Rating Descriptive Statement

OutstandingApplicant fully addresses all aspects of the criterion, convincingly demonstrates that it will meet the City’s performance requirements, and demonstrates no weaknesses
GoodApplicant fully addresses all aspects of the criterion, convincingly demonstrates a likelihood of meeting the City’s requirements, and demonstrates only a few minor weaknesses.
AdequateApplicant addresses all aspects of the criterion and demonstrates the ability to meet the City’s performance requirements. The Response may contain significant weaknesses and/or a number of minor weaknesses.

Poor Applicant does not address all aspects of the criterion nor is evidence presented indicating the likelihood of successfully meeting the City’s requirements. Significant weaknesses are demonstrated and clearly outweigh any strengths presented.

Unacceptable Applicant does not address all aspects of the criterion and the information presented indicates a strong likelihood of failure to meet the City’s requirements.

Green Acceptable Response - Applicant addresses all aspects of the criterion and demonstrates the ability to meet the City’s performance requirements. The Response may contain significant weaknesses and/or a number of minor weaknesses.

Yellow Marginal Response - Applicant does not address all aspects of the criterion nor is evidence presented indicating the likelihood of successfully meeting the City’s requirements. Significant weaknesses are demonstrated and clearly outweigh any strengths presented

Red Unacceptable Response - Applicant does not address all aspects of the criterion and the information presented indicates a strong likelihood of failure to meet the City’s requirements.

Sample C:

Only the following numerical rating values may be assigned: 10, 8, 5, 2, and 0. The scoring of each criterion must be based on the strengths and weaknesses of the Response. To assist in assigning an appropriate score, the following can be used as a guideline:

RatingDescriptive Statement
10Applicant fully addresses all aspects of the criterion, convincingly demonstrates that it will meet the City’s performance requirements, and demonstrates no weaknesses.
