About this file

This is a Vote-by-Mail (VBM) Ballot Application form for the Cuyahoga County Board of Elections in Ohio, applicable to the November 2026 General Election. The form serves as the primary document through which registered voters can request absentee ballots for voting by mail. Applicants must complete all five required sections: name and address, date of birth, identification information (last four Social Security number digits, Ohio driver's license/state ID number, or copy of valid photo identification), election selection, and signature. The application must be received by the Cuyahoga County Board of Elections no later than close of business seven days before Election Day, either by mail to P.O. Box 89448, Cleveland, OH 44101-6448, or in person at 1803 Superior Avenue, Cleveland, Ohio 44114. Once processed, the Board mails ballots to the applicant's provided address. Voters may track application and ballot status online at boe.cuyahogacounty.gov or by calling 216-443-8683.

Completed ballots must be received by 7:30 p.m. on Election Day through one of three methods: mail delivery with postage, drop box submission without postage, or in-person delivery to the Board of Elections office without postage. Only the voter or qualified near relatives may deliver ballots, except voters with disabilities may designate any person except employers or union officers; ballot deliverers must complete Form 12-P attestation at the Board office. Members of uniformed services and overseas voters have until the fourth day after the election for ballot receipt. The form includes instructions in both English and Spanish and offers a poll worker interest option. No pricing, funding, or contract-specific terms are identified in this application form, as it functions as a standard voter service document rather than a procurement solicitation.
