About this file

This is a Request for Quotation (RFQ) issued by the Ohio Department of Natural Resources (DNR) for Salt Fork State Park, located in Lore City, Ohio. The document solicits mining rough materials specifically designed to fit the existing Sandy Creek Mining Company sluice at the park's campground. The project requires precise mining rough sizing to ensure compatibility with the current sluice system, with a completion/delivery date of April 24, 2025. The contract will be awarded to the lowest responsive and responsible bidder, determined by calculating the low lot total through multiplying unit costs by estimated usage. The contract term extends through June 30, 2025, with a requirement that any goods or services after this date will necessitate a new purchase order.

The RFQ includes detailed invoice submission requirements for Financial Shared Services (FSS), specifying two submission methods: email (invoices@ohio.gov) or mail. Invoices must include 11 specific elements, such as the purchase order number, supplier name, ship-to address, unique invoice number, service date, itemized charges, and line item breakdowns. Notably, no Ohio state sales tax will be applied, and the invoicing process is stringent, with incomplete submissions resulting in invoice returns and payment delays. The document emphasizes precise documentation and adherence to state procurement guidelines for this state park infrastructure procurement.
