SUBJECT: Safety Office Review of Portable 2000-watt Laser Ablation System with Accessories and Spare Parts.

1. Purpose: LEAD Safety Office review of Portable Laser Ablation System per PD received on 11-15-23.

The Equipment, Accessories, and parts include, but is not limited to the following details:

Portable laser ablation system: 2000 watts (with the ability to adjust down to 200 watts) continuous wave beam source average power, random polarization, pulsed, hermetically sealed resonator, Class 4 laser product, includes:

- Wavelength: 1064nm

- One (1) hand-held laser optic

- Input power required: 220VAC, 3-ph, 40A, 50/60Hz

- Built-in water-cooling unit

- Operating temperature: 32-104°F non-condensing

- Humidity: 30-85% non-condensing

- User Manuals, digital & hardcopy

- Compact accessory kit consisting of lens cleaning set and Input/Output connectors

- Dehumidifier/air dryer unit

Laser Source Housing Certified to ANSI Z136, NFPA-701, Material rated to Class IV protection, Limit Switches/ Door Lock to cut off power to laser when barriers are opened.

Laser Safety Curtains consisting of:

- Two (2) foldable laser light protective 6-panel rigid systems (25ft long x 7ft tall)

- Eight (8) portable 5M collapsible laser curtains, 6ft W x 7ft H (similar to Kentek part number PT-5M250WB6) with Velcro curtain-to-curtain edge-sealing

- One (1) laser light protective pop-up canopy tent with floor length side panels (10ft X 10ft). Includes a durable reusable stowage/carrying case.

- All curtains and tent shall include laser safety signage

SUBJECT: Safety Office Review 2000-watt Laser Ablation System 23 October 2023 for Letterkenny Army Depot

User Interface Controls

Hand-held Laser Optic

Fiber Optic cable Package, Laser Safety Glasses (meet or exceed ANSI/ASEA Z87.1), Accessories, and spare parts.

The requestor is responsible for ensuring the adequacy of the item(s) for the intended use or purpose of the request prior to submittal for safety review.

2. The following information shall be added to the PD/PWS. If there is no PWS for the contract or the PD/PWS has already been approved, the contractor shall be notified of the following items.

a. The contractor is reminded that LEAD maintains a Safety Management System

(SMS) and is ISO 45001 certified, AS9100 and AS9110 certified.

b. LEAD’s safety policy and safety program (LEAD-R 385-1 and Safety Bulletins) can be found at https://letterkenny.army.mil/visitors-Information.

c. It is the contractor’s responsibility to be familiar with theses documents and to comply with their contents.

d. Contractor/Subcontractor will conduct work on Letterkenny Army Depot in compliance with the most current version of 29 CFR 1910 and 29 CFR 1926 (where applicable). Each contractor/subcontractor will be familiar with the types of work they are expected to perform and each particular phase of the operation and associated hazards. All worksites will be free of recognized hazards (per LEAD-R 385-1 27-2).

i. Each worksite will be assessed for hazards prior to the start of work.

All training certifications and an Activity Hazard Analysis (AHA) will be on hand at the job site.

ii. Any and all on site work at LEAD shall be completed in accordance with applicable OSHA, DoD, DoDI 6055, Army, and LEAD Safety requirements. As a federal installation, additional requirements other than OSHA apply to contractors performing work at LEAD. Examples include: full conformance to all applicable NFPA standards, ANSI standards (including ANSI/ISEA Z87.1), and other consensus standards for the industry covered by the subject contract. It is the responsibility of the contractor to be familiar with all applicable standards prior to starting work. Additionally, the contractors shall make themselves familiar with and comply with LEAD's local safety regulation LEAD-R 385-1, AR 385-10, DA PAM 385-24, and LEAD-R 420-13 prior to the start of any work at LEAD.

iii. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as hose created by point of operation, nip/pinch points, rotating parts, flying chips, and sparks. The guarding device shall be in conformity with LEAD-R 385- 1 and any appropriate standard (OSHA, ANSI, Equipment Owner’s Manual.

iv. In case of a conflict among these laws and regulations, the most current and stringent law or regulation shall apply. It is the responsibility of the contractor to be familiar with all applicable standards prior to starting work.

v. A list of all hazardous materials will be maintained at all times and Safety Data Sheets (SDS) will be present and maintained at all worksites. A copy of all SDS’s will be provided to the Safety Office at the beginning of all projects.

vi. SDS’s will be provided to the Safety Office at the beginning of all projects.

e. The 2D Digital Radiography (X-Ray)/Computed Tomography (CT) system and enclosure shall comply with OSHA 21 CFR 1020.40.

f. Equipment installation/set-up, and testing shall be conducted accordance with the equipment manufacturer specifications, and according to the frequency recommended by the manufacturer.

g. The coordination of the equipment installation shall be schedule with the COR.

h. The Contractor shall ensure all employees and all associated sub-contractor’s employees possess all required licenses for operating any equipment used in the performance of this contract. The Contractor must ensure that any personnel performing under a contract are fully trained, licensed, certified, and qualified for the position in which they shall be serving.

i. All contractor personnel shall wear the appropriate PPE when in the following work areas: industrial buildings, at construction sites, test sites and test facilities. Failure to wear/use PPE may cause removal from the job site and/or seizure of non-compliant equipment/items.

j. If contractor personnel determine a procedure is unsafe, they shall discontinue work immediately and contact their Program Manager who shall notify safety personnel and the COR. As soon as LEAD Safety Office personnel determine that the unsafe conditions have been corrected, the COR will notify the contractor Program Manager that personnel may resume work.

k. The contractor shall comply with reporting requirements of 29 CFR Part 1904 and AR 385-40 and shall report accidents resulting in injury or death and accidents resulting in damage to Government property, supplies, and equipment to the COR promptly of the occurrence. In case of injury/accident, the contractor shall complete an injury/accident investigation report and submit it to the COR with a copy to the LEAD Safety Office within 24 hours of the event, regardless of if the event is reported to another Safety Management System (SMS).

l. Directorate of Contracting will ensure ergonomic designs are considered in all contracts for the purchase of new equipment and tools.

m. The Awarding Contracting Office will:

i. Require all suppliers of chemicals to provide current SDSs with each shipment as set forth by the Federal Acquisition Regulation (FAR), Federal Standard 313V Appendix A, and OSHA 29 CFR 1910.1200.

ii. Ensure all procured HazMat is restricted to products that have been approved for use by the HMAG, to include any HazMat provided with equipment or items purchased (e.g., lubricant grease or engine oil).

iii. Pursue sanctions against vendors who consistently do not provide proper SDS.

iv. Require all contractors to provide a completed AMLD Form 4783 LEAD Contractor Chemical Checklist and include SDSs for all chemicals to be brought onto the Depot prior to performing work on-site.

n. Requisitioning, Purchasing, or Otherwise acquiring HazMat: All HazMat as defined by LEAD-R 385-1, Safety Bulletin 23-02 shall be approved using an HMAG request prior to being acquired, received, and issued through an IRP.

o. Obtaining Approval to Requisition, Purchase, or Use a New HazMat will follow LEAD-R 385-1, Safety Bulletin 23-02, 10.6.b

p. All safety messages or recalls related to the safe use of products provided by the contractor will be provided to the LEAD POC for the Contract within 5 days of contractor’s awareness of the message or notice.

3. The COR will notify all local work area supervisors prior to actual work to ensure employee safety.

4. The Directorate HMAG coordinator is the point(s) of contact within a Directorate or equivalent organization responsible for facilitating HazMat validation, ensuring accurate HMAG Request Details on the for, and resolving nonconcurrence issues that may arise during the HMAG review.

5. If applicable to the equipment and/or service purchased an approved Request for Change (RFC) shall be completed prior to contract solicitation.

6. Purchased equipment and/or services that include training shall be reported to the LEAD Skills Develop Office (SDO).

7. If the requirements for this contract change, the proponent of the contract must submit all changes to the LEAD Safety Office for review.

8. COR/LEAD POC shall send a copy of the LEAD contract for this purchase to the Safety POC listed below within 30 business days of contract award.
