About this file

This is a Pricing Schedule Excel spreadsheet for a VA Medical Center Request for Quote (RFQ) for medical surgical items. The solicitation (36C77025Q0198) is for two line items: a Heat & Moisture Exchanger (Provox #8262) and an Intermittent Catheter Set (14FR), to be delivered to the Charleston and Chelmsford Consolidated Mail Outpatient Pharmacies (CMOPs). The quote is due on 6/13/25 at 3:00 PM Central Time, with a 10-day delivery requirement after award.

Key submission requirements include completing the SF1449 cover page, providing the quote spreadsheet, submitting OEM authorized dealer documentation, and including a Buy American Act Certificate. Vendors must provide pharmaceuticals and medical surgical supplies with expiration dates of at least one year from delivery. The solicitation emphasizes strict gray market prevention, requiring vendors to be authorized OEM dealers with documentation verifying their status, and prohibiting remanufactured or unauthorized channel items. The procurement is for the Department of Veterans Affairs Veterans Health Administration, specifically Veterans Integrated Service Network 15.

Gray Market Prevention: Deviation to policy allows non-manufacturing vendors selling medical surgical supplies to satisfy the Gray Market Prevention language by providing letters of authorization from a manufacturer's authorized wholesaler/reseller via an authorization letter directly from the manufacturer.

(a). Gray market items are Original Equipment Manufacturer's (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactured or gray market items will be accepted.

(b). Vendor shall be an OEM authorized dealer or authorized distributor for the proposed medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e. replacement parts). Verified by an authorization letter or other documentation from the OEM, such that the OEM's warranty and services are provided and maintained by the OEM. All software licensing, warranty and services associated with the medical supplies, medical equipment, and/or services contract for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.

(c). The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. this includes termination of the contract or, solely at the VA's election, allowing the vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
