About this file

This document is a Past Performance Questionnaire for a Veterans Affairs lease opportunity in Salisbury, Maryland. The five-page form is designed to collect detailed performance feedback from previous contract assessors, covering seven key evaluation areas: quality of service, schedule/timeliness, customer satisfaction, management/personnel, cost/financial management, safety/security, and general performance. The questionnaire uses a rating scale from Exceptional (E) to Unsatisfactory (U), with assessors asked to provide granular ratings across multiple subcategories and the option to include written comments. The form is part of the solicitation process for NCO5 Network Contracting Office's requirement to secure leased space for a Veterans Center, with evaluators expected to thoroughly document the contractor's historical performance across multiple dimensions to help inform the source selection decision.
