STATEMENT OF WORK
Heating Hot Water Pipe Repair in Mechanical Rooms
1. Scope:
Contractor shall repair heating hot water pipe connections in four mechanical rooms, located at the Louis Stokes Cleveland VA Medical Center, 10701 East Blvd; Cleveland, OH 44106.
Contractor shall visit the site to determine the current piping layout prior to submitting a quote.
Installation to begin immediately upon award and to be coordinated with the VA Point of Contact (POC) listed below.
The period of performance is 180 calendar days.
2. Background:
The existing pipe connections are failing and leaking. The result is difficulty in maintaining the air temperature in patient care areas of the hospital. The repair shall restore the piping to its original water-tight condition.

4. Specific Tasks:
Provide all materials, supplies, equipment, labor, and supervision required to repair the heating hot water pipe connections in the following locations:
a. 1F-532, PALMS Research
b. BF-504, Back of Warehouse
c. 2B-480, East Penthouse
d. BF-192, Mechanical Room
The repair shall consist of the installation of a grooved, gasketed compression coupling at each pipe connection to meet plumbing code requirements.
Performance standard for coupling is Victaulic 107V Rigid and 177N Flexible Couplings.
Grooved Steel Piping System:
Grooved steel piping system shall be used in lieu of welded or screwed joints.

Model numbers are based on Victaulic Company.
Grooved joint couplings and fittings shall be specifically identified with the applicable style or series designation.
Pipe Couplings:
Consisting of two pieces of ductile iron.
Coupling gaskets shall be an “EHP” EPDM synthetic rubber gasket with a central cavity pressure responsive design.
Coupling bolts and nuts shall be heat treated carbon steel, trackhead conforming to chemical and physical properties of ASTM A449 and physical properties of ASTM A183.
a. Rigid Type:
Housing shall be cast with torque-absorber and shift-limiting slant bolt pad design to provide system rigidity and support and hanging in accordance with ASME B31.1 and B31.9, and to prevent mis-assembly due to over shift.
Housings cast with alignment indicator notch for visual proper alignment to adjoining fitting.
1) 2" through 12” sizes: 107V “Quick Vic” Installation Ready
b. Flexible Type:
For use in location where vibration attenuation and stress relief are required.
1) 2" through 8" sizes: 177N "Quick Vic" Installation Ready
2) 10" through 12" sizes: Victaulic Style 77
5. Performance Monitoring:
The VA POC and VA Plumbing Shop will monitor contractor performance.
6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI):
None.
7. Inspection and Acceptance Criteria:
The POC shall make the final acceptance determination using manufacturer requirements and recommendations.
8. Security Requirements:
Contractor employees providing services at VA facilities may undergo a security clearance in the form of a basic background investigation.
9. VA Sensitive Information:
Dedicated personnel will be assigned solely to observe, interact, assist, and assure protection of VA sensitive information.
10. Contractor-Owned IT Devices:
The C&A requirements do not apply, and a Security Accreditation Package is not required.
11. VA Medical Device Protection Program (MDPP):
Service & Maintenance will follow the security requirements as per VA’s Medical Device Protection Program (MDPP) which protects VA’s medical devices through a comprehensive security initiative that encompasses pre-procurement assessments, medical device isolation architecture (MDIA), communication, validation, scanning, access control list remediation, patching, and secure remote connectivity.
12. General Security:
Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.
13. Security Check-In Requirements:
The Contractor Service Personnel and/or representative must report to the appropriate department Supervisor or designated point of contact (POC) to sign in with department staff before work begins. Additionally, one must submit any mobile media devices that would be used on the system for a virus scan. Upon completion of work, the Contractor Service Personnel and/or representative must report to the appropriate department Supervisor to brief that Supervisor or related department staff point of contact (POC) if Supervisor is unavailable concerning completion of service, and then to sign out. At the end of each briefing, Contractor Service Personnel and/or representative will and must sign out.
14. VA Information Custodial Language:
Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor/subcontractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA.
This clause expressly limits the contractor/subcontractor's rights to use data as described in Rights in Data - General, FAR 52.227-14(d) (1).
15. Security Incident Investigation:
a. The term “security incident” means an event that has, or could have, resulted in unauthorized access to, loss or damage to VA assets, or sensitive information, or an action that breaches VA security procedures.
The contractor/subcontractor shall immediately notify the COTR and simultaneously, the designated ISO and Privacy Officer for the contract of any known or suspected security/privacy incidents, or any unauthorized disclosure of sensitive information, including that contained in system(s) to which the contractor/subcontractor has access.
b. To the extent known by the contractor/subcontractor, the contractor/subcontractor’s notice to VA shall identify the information involved, the circumstances surrounding the incident (including to whom, how, when, and where the VA information or assets were placed at risk or compromised), and any other information that the contractor/subcontractor considers relevant.
