DESCRIPTION

This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services,” as supplemented with additional information included in this notice.

This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.

This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-02, effective 12/22/2023.

This solicitation is set-aside for: 100% Service Disable Veteran Owned Small Business

The associated North American Industrial Classification System (NAICS) code for this procurement is 811210 with a small business size standard of $34 Million.

The FSC/PSC is J065.

The VA Greater Los Angeles Healthcare System (VAGLAHCS) is seeking a contractor to certified and licensed contractor to inspect and repair medical air, oxygen, dental air, operating nitrogen, medical vacuum, monitoring and alarm systems for medical gases.

After the repairs, the contractor shall also provide certification for use. Technicians are required to be ASSE 6040 certified. Contractor shall also provide emergency on call repairs as needed. Services will be provided at the West Los Angeles Medical Center (WLA VAMC), 11301 Wilshire Blvd, Los Angeles, CA 90073-1003, the Los Angeles VA Clinic (LA ACC), 351 E Temple St., Los Angeles, CA 90012-3328, and the Sepulveda VA Medical Center (SACC), 16111 Plummer St., Sepulveda, CA 91343-2036.

The award will be made to the quote most advantageous to the Government and best meets the requirement described in the statement of work.

The following are the decision factors: Price, and past performance.

FAR Clause 52.219-75 Certificate of Compliance for Services and Construction is REQUIRED to be submitted at time of quote submission.

STATEMENT OF WORK (SOW)

1. Objective

The objective of this service contract is to have a contractor to all labor, materials, supplies, tools, equipment, parts, software (needed for the general operation of the equipment), travel, and qualified personnel to provide medical gas services, which include medical air, oxygen, dental air, operating room nitrogen, medical vacuum, monitoring and alarm system for these critical medical gases, as per below scope.

2. Scope:

The contractor shall provide all labor, materials, supplies, tools, equipment, parts, software (needed for the general operation of the equipment), travel, ICRA Barriers, supervisors, and qualified personnel to accomplish the tasks outlined in this SOW. Contractor is responsible for all subcontractors required to complete reports.

All work shall be performed during normal hours unless otherwise approved by the POC or his/her designee.

3. Contractor Qualifications & Other Responsibilities

Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees’ fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. Copy of licenses are to be provided at time of quote/proposal.

The Government reserves the right to accept or reject Contractor’s employee for the rendering of services. Complaints concerning Contract Personnel’s performance or conduct will be dealt with by the Contractor and POC with the final decision being made by the Contracting Officer.

Contractor to provide service to all medical gasses’ locations listed:

Medical Gas System Maintenance/ Testing for GLA VA Medical Center:

Medical air filter changes in the interstitial of building 500.

Dental vacuum line flushing every 6 months.

Vaporizer changes on the bulk oxygen tank system Weekly.

Service to medical air compressors, Instrument Air compressor, Vacuum Pumps and

Dental Air compressors. Location of these units is building 501. Report of annual service rendered shall be provided.

A technician is required to adequately perform tasks on-site at least three days a week.

Contractor shall have multiple ASSE 6040 certified technicians (minimum of six techs) located within 50 miles and able to provide emergency response services a reasonable time frame as determined by COR)

Contractor to be awarded shall also have multiple ASSE 6030 (minimum of three techs) There shall be a 24 hour/day 7days a week response to emergency service needs to include all holidays.

Annual preventative maintenance inspection report shall be provided to the COR, using the most current applicable NFPA 99 as a guideline. The reports shall include a system performance summary, source evaluation report according to applicable NFPA 99 CODE, and pictures of the following systems:

Oxygen, Medical Air, Vacuum, Nitrous Oxide, Nitrogen, Carbon Dioxide.

Perform Annual Medical Gas Inspection to include inspecting all Source Equipment (Bulk Oxygen system, Medical Air Compressors, Vacuum Pumps, Nitrous Oxide Manifold, Nitrogen Manifold, Carbon Dioxide Manifold), Zone Valves, Master Alarm Panels, Area Alarm Panels, Wall Outlets. Provide a full detailed report with all findings.

Perform Repairs to all discrepancies found during the Annual Inspections. Repairs are to be completed within 45 days of inspections. All repair parts to include “O-rings”, faceplates, valve bodies, labels, zone valve gauges. Provide a full detailed repair report.

Perform Semi-Annual flushing of Dental Vacuum Lines with cleaning fluid. Provide a detailed report listing all locations that were flushed.

Check Nitrous Oxide Manifold bank pressures twice weekly. Record pressures and change cylinders as needed. Provide report as work is performed.

The award will be made to the quote most advantageous to the Government and best meets the requirement described in the Statement of Work.

Submissions shall include your quote, using the attached schedule, as well as a capabilities statement with qualifications proving all certifications and education required for the work being conducted in the SOW. Submissions not meeting all requirements of the SOW will be considered “ineligible for award”. A filled-out FAR 52.219-75 MUST also be included in the submission or it will be considered “ineligible for award”.

Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212- 1(f).
