About this file

The document is a Surveillance Checklist for a Ground Ambulance contract at the Tomah VA Medical Center (VAMC) in Wisconsin. The checklist outlines six key performance objectives with 95-100% compliance requirements, focusing on 24/7 ambulance service, vehicle maintenance, personnel qualifications, and medical training standards. Specific surveillance requirements include 100% monitoring of patient pickups, ensuring vehicles are clean and safe, maintaining detailed vehicle maintenance records, and verifying that personnel have completed a minimum 150-hour training program approved by the Department of Health and Human Services.

The related federal contract opportunity is a non-emergency ambulance services solicitation (36C25225Q0652) issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 12, with a 5-year ordering period from September 1, 2025 to August 31, 2030. The surveillance checklist provides detailed performance expectations and quality assurance methods that would likely be used to evaluate contractor performance during the contract period, with stringent requirements for service delivery, safety, and medical personnel qualifications.
