Lovell Federal Healthcare Center. The procurement is for an Orion Comprehensive Rotary Chair set aside for Service Disabled Veteran Owned Small Businesses (SDVOSB), to be awarded as a firm fixed-price purchase order. The solicitation (36C25225Q0318) is a combined synopsis/solicitation for a one-time purchase, seeking quotes from authorized dealers for specific items including the rotary chair, software, printer kit, database, installation, and on-site training. The procurement is limited to SDVOSB vendors, with a NAICS code of 334510 and a small business size standard of 1,250 employees. Quotes are due by 3:00 p.m. Central Standard Time on March 20, 2025, and will be evaluated comparatively based on price, technical merit, veterans involvement, and speed of delivery. VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Single Source Justification for SAP under the SAT

Last Updated: 02/01/22 Page 1 of 2

DEPARTMENT OF VETERANS AFFAIRS

Justification for BRAND NAME Awards IAW FAR 13.106 1 For

Over Micro Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C252 25 AP 1850/ 556 25 2 2993 0117

1. Contracting Activity: Department of Veterans Affairs, VISN 12, Capt. James A Lovell FHCC has submitted 556 25 2 2993 0117 for Orion Comprehensive Rotary Chair to the Great Lakes Acquisition Center (NCO12). Heather Kamath will be the Lovell FHCC contact point & Jennifer Mead from NCO12 will be responsible for the Contracting Action.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This justification and approval is for a Brand Name Orion Comprehensive Rotary Chair procurement which will be set a side for Service Disabled Veteran Owned Small Businesses and awarded as a firm fixed priced purchase order. This system is used to assess vestibular function.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Lovell FHCC requests the Interacoustics Orion Auto Traverse Rotary Chair with VisualEyes 525 software to serve as a replacement for their current end of life rotary chair equipment. The new equipment will allow the clinician to efficiently evaluate Veterans for inner ear pathology, using oculomotor testing, sinusoidal harmonic acceleration, step velocity, and dynamic and static subjective visual vertical, and off axis centripetal acceleration. The full enclosure will work well in the current space, as the end of life rotary chair that is being replaced is also fully enclosed. Full enclosure is a required characteristic of this equipment in order to perform full field optokinetic testing as part of the rotary chair oculomotor test battery. Updated testing and interpretation software, included with the equipment purchase, will allow for additional vestibular ocular reflex measurements (eg: eye torsion) to be made which are not possible with our current equipment. All of the other vestibular test equipment at FHCC is the same Brand/Model (Interacoustics) and software (VisualEyes). It is not safe practice to have multiple models, and having the same brand name/model allows for more expedited repairs by BioMed as parts can be interchangeable if one goes down for repair. This, in turn, provides better access to care and reduced wait times when there is a need for repair because appointments do not need to be cancelled when parts can be interchanged. Lastly, all other FHCC software for vestibular testing is software (VisualEyes), the training provided for this equipment carries over to the software for the other vestibular diagnostic equipment, making the cost of training more effective with brand name only. Note that Interacoustics is the ONLY BRAND that manufacturers VisualEyes software products, which is a salient required characteristic of the equipment.

Installation and Training is required.

4. Description of market research conducted and results or statement why it was not conducted:

Two VetCert listed SDVOSBs were located who indicated they were capable of meeting the requiement and able to submit a quote at a fair and reasonable price that offers the best value to the government. Both SDVOSBs indicated they were authorized distributors of the OEM. The OEM, Interacoustics, is an other than small business. This NAICS code is on the NMR class waiver list.

Therefore, the chairs will be procured using a Brand Name RFQ on Open Market with an SDVOSB set aside. The products are produced in the U.S. satisfying the Buy American Act.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106 1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
