About this file

This is a Buy American Certificate provision required as part of the solicitation for the Veterans Healthcare Administration Salt Lake City Health Care System (VHASLCHCS) Ostomy and Gastrostomy Supplies Indefinite Delivery Indefinite Quantity (IDIQ) contract, issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 19. The certificate mandates that offerors certify whether each end product is a domestic end product or a foreign end product in compliance with Federal Acquisition Regulation Part 25 Buy American requirements.

Offerors must list all foreign end products manufactured in the United States that do not qualify as domestic end products, and for foreign end products not consisting wholly or predominantly of iron or steel, indicate whether they exceed 55 percent domestic content (except for commercially available off-the-shelf items). Additionally, offerors must separately identify line item numbers of domestic end products containing critical components as defined in the solicitation's Buy American-Supplies clause. The Government will evaluate all offers according to Part 25 policies and procedures. This is a template document requiring offerors to complete the foreign end products and critical component sections during proposal submission.
