About this file

This is a Performance Work Statement for fire system maintenance at the Hot Springs Veterans Affairs Medical Center (VAMC) in Hot Springs, South Dakota.

The project encompasses two primary work components: (1) excavation and replacement of two gate valves with new post indicators on fire mains adjacent to Buildings 66 and 1, with one 6-inch valve at Building 1 and one 4-inch valve at Building 66, both featuring mechanical joint type connections at depths of approximately 5 to 6 feet; and (2) removal and replacement of one 90-minute rated metal double fire exit door and frame near the Sterile Processing Service area. The contractor must supply all materials and labor, provide new replacement gate valves with post indicators, handle excavation requiring trench boxes or shoring due to proximity constraints, complete testing and grading, and install the fire exit door with all hardware painted to match existing aesthetics. The total period of performance is not to exceed 30 calendar days from Notice to Proceed, with on-site physical work limited to 11 days maximum. Contractor personnel must complete VA Privacy Training (Course ID 20939) and Infection Control (ICRA) training within one year prior to the NTP start date and attend a pre-work Patient Privacy/Information Security briefing lasting no more than 30 minutes. All work must meet applicable hospital environment codes and standards, with finishes aesthetically correct and work areas cleaned and returned to original condition daily and upon project completion. The contract contact is located at Hot Springs VAMC, 500 North 5th Street, Hot Springs, SD 57747.

Part of the work consists of excavating and replacing two gate valves and their post indicators on the current fire mains adjacent to Building 66 and Building 1. Photos of the post indicators and work areas for the two PIV valves are included.

The other part of the work consists of removing and replacing one 90-minute double fire exit door and frame at the end of a main hallway. Images of the door are also included.

Location of Work

This work is located entirely within the HS-VAMC campus.

The mailing address for this project is:

Hot Springs VAMC 138-HS — J. Thomason 500 North 5th Street Hot Springs, SD 57747

The physical address for this work is: 500 North 5th Street, Hot Springs, SD 57747

Period of Performance

This work is expected to take no more than 30 calendar days from the Notice to Proceed (NTP), including all lead times on equipment and material. The physical on-site work is expected to take no more than 11 days. See section 01 30 13 Project Coordination in the General Requirements portion of this contract for explanation.

General Requirements

Contractor personnel are required to have the VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access to or the Use of VA Sensitive Information (Course ID 20939, attached) and Infection Control (ICRA) training within the previous year of the start date of the NTP and will be required to attend a Patient Privacy/Information Security briefing prior to starting work. This briefing will take no more than 30 minutes. The briefing may be combined with the initial on-site pre-work meeting. This training is in addition to the applicable OSHA safety and task training appropriate for this work.

See also “Contract General Requirements” section.

Deliverable(s)

The contractor shall provide all aspects of work and material to accomplish both parts of the work of this contract.

See attached Figures (10 images) for more information.

Part One: PIV valves

The work is to replace in kind with new valves to identical specifications of those currently installed. Existing valves, once removed, shall be provided to the COR for evaluation for repair or refurbishment.

Statement of Work Pg 2 of 3

The first part of this work is the replacement of two gate valves: replace one 6” gate valve at the base of the post indicator (PIV) adjacent to Building 1 and the replacement of a 4” gate valve at the PIV adjacent to Building 66. Both valves are mechanical joint type connections.

The contractor shall supply and install two new replacement gate valves with new post indicators on the plumbing branches feeding the fire suppression system in two buildings. The valves will require excavation to expose, replacement, testing, fill & compact, and installing new minor hardware (fasteners, etc.) as required to install the new valves and indicator posts, and regrade disturbed ground.

Layback or ‘stepping’ of surrounding material for safety will not be possible due to close proximity of equipment and improvements. Trench box or other means of shoring will be required. Excavation walls may need to be braced during excavation to prevent damage to adjacent equipment and sidewalk.

The PIV valves are expected to be approximately 5’-6’ feet deep.

Part Two: Fire Exit Door

This part of the work will take place near the Sterile Processing Service portion of the Hospital and therefore, extra precaution for dust, debris and contamination control shall be exercised.

The second part of the work is the removal of existing and replacement in kind of an existing fire rated double door and frame with one 90-minute rated metal double fire door and rated metal frame. See sections 08 11 13 for door and frame requirements and sections 08 71 00 for hardware requirements.

Existing door and frame, including frame mounting brackets and hardware, to be removed and disposed offsite.

Existing wall surface shall be cleared of all existing mounting hardware and fasteners and prepared for mounting new frame. Existing walls at mounting locations on bottom of frame on both sides may need to be cleared and cleaned of rust and/or damaged and/or swollen concrete. Any existing rusted hardware fixed to the concrete wall shall be removed, replaced and painted with an oil based enamel or epoxy in order to prevent spread of rust after the work is finished.

Some swelling of the existing block, concrete, or grout of the wall may need to be resurfaced and squared up to properly mount the new frame. This seems likely for a few inches at the bottom of the doorway but may occur elsewhere on the mounting surface to a lesser degree. The contractor shall be familiar and capable with methods of performing rust removal and treatment, and minor abrading of concrete surfaces to provide a sound and suitable mounting surface.

Equipment for such should be brought to ensure completion of work in a single session. Contractor is cautioned to provide strict control of dust and debris for this part of the work.

Replacement shall include all hardware. The frame, frame mounting hardware, and doors shall be painted to match the general aesthetic of the area in which they are installed. Mounting hardware and brackets that are not made of non-corrosive materials shall be painted with a durable, compatible enamel and allowed to dry thoroughly prior to using them in the work. Paint shall be re-applied on-site where paint finish is damaged prior to finishing the installation. Exposed, visible portions of the door shall be painted with matching paint, invisible portions (i.e., inside the frame) may be painted with any color of compatible, durable enamel paint.

Statement of Work Pg 3 of 3

Performance Standard

All items of this contract shall be performed to the highest professional standards, complete and tested according to applicable codes and standards normal for this work in a hospital environment. The electronic indicator system (for PIV valves) shall also be reconnected, fully operational, and ready for use at the end of this work. Finishes and details shall be aesthetically correct (matching) and in excellent condition at the finish of the work. The work areas shall be cleaned and returned to their original condition at the end of each day (to the extent practical) and at the end of the project. At the end of the project, the ground around the new PIV valves shall be graded (by hand or machine) where damaged or excavated during the project, seeding and mulching will be done by others.
