B.2 STATEMENT OF WORK (SOW)

Background: The Dental Services at the VA San Francisco Health Care (Dental) located at 4150 Clement St, San Francisco, California 94121 and at the South Santa Rosa VA clinic located at 2285 Challenger Way, Santa Rosa, California 95407 has a requirement for a full-service dental laboratory to provide fixed, removable, and implantable dental prostheses for Veterans. Contractor may also be required to attend patients’ appointments for services that include but are not limited to; photogrammetry-driven full arch work flows and custom-made maxillofacial prostheses.

3. Special/Significant Requirements and Services: Geographic Accessibility and Past Performance

a. Some circumstances may dictate the need for a dental order to be processed and returned within a shorter timeframe, such as a same-day repair, reline, add teeth of a removable prosthesis. There may also be circumstances where a patient needs to travel to the dental lab for custom shade matching/staining. For these reasons, the contracted dental lab must be located within 60 miles of both the VA San Francisco Dental Clinic and the South Santa Rosa VA Dental Clinic.

Given the average age (70+), their complex health states and distance traveled-sometimes 3+ hours or flown in from out of state, this accessibility will mitigate hardship upon Veterans in the delivery of quality care outcomes.

As determined by the VA San Francisco Dental Clinic and the South Santa Rosa VA Dental Clinic, the Contractor may be required to attend a patient’s appointment, possibly on short notice, when there is an urgent situation or complex implant procedure to provide dental supplies and/or provide assistance with digitizing patient records and capturing implant locations utilizing advanced photogrammetry technology.

c. VA San Francisco is a teaching institution; therefore, the contracted laboratory should provide continuing education courses geared towards resident education.

The facility should be within a 60-mile driving radius so that residents can attend these courses in person. Also, the contractor will be asked to provide a tour of their facility to showcase their work, demonstrating knowledge of digital and complex workflow and to provide affirmation that the stated products are indeed being manufactured at their laboratory site.

4. General Requirements

a. The contractor will provide laboratory support for removable and fixed prosthodontic laboratory work for the San Francisco VA and South Santa Rosa VA Dental clinics. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s QCP is the means by which the Contractor assures that their work complies with the requirements of the contract.

Within the QCP, the Contractor shall propose and maintain an established reporting system to promptly notify VA of any problems, issues, risks, or concerns with workflow performance, programmatic improvement, security, and schedule adherence. The QCP shall be submitted in written form with an electronic copy to the Point of Contact (POC) for review within thirty (30) calendar days of the contract award. The POC or designated representative will review the QCP and return it to the Contractor if changes are required. If any changes are required, the Contractor has five (5) working days to complete the changes and resubmit the QCP. If any future changes are needed, the Contractor has five (5) working days to submit the agreed upon changes. Quality control is the exclusive responsibility of the Contractor.

It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s). The QASP will help to ensure that service levels reach and maintain the required levels throughout the contract term. Further, the QASP provides the POC with a proactive way to avoid unacceptable or deficient performance and provides verifiable input for the required Past Performance Information Assessments. The QASP will be finalized immediately following award and a copy provided to the Contractor afterward. The QASP is a living document and may be updated by the Government as necessary.

The QASP will be provided to the Contractor after award of contract. The POC will monitor Contractor performance. Documentation of services performed will be reviewed prior to certifying payment.
