About this file

This is an Evaluation Worksheet and Conflict of Interest Certificate for a Request for Proposal (RFP) issued by the Oregon Military Department for Medical Services for Firefighters at the Portland Air National Guard Fire Department. The document serves as the standardized form that evaluation committee members must complete when assessing proposals submitted in response to Solicitation Number S-24800-00015803. The worksheet contains two phases of evaluation: Phase 1 assesses minimum responsive requirements including company information, qualifications, references, legal entity status, affirmative action compliance, SAM exclusion verification, sample contract certification, personnel resumes, and total cost proposals; Phase 2 evaluates proposals that pass Phase 1 based on experience in providing physicals and medical services to fire protection personnel (50 points), experience in providing written medical evaluation reports (40 points), and total cost (10 points), for a total possible score of 100 points.

The evaluation worksheet requires committee members to complete a mandatory Certificate of Conflict of Interest before beginning their independent assessments, certifying they have no actual or undisclosed potential conflicts with any responding proposer, have not communicated with proposers since the RFP issuance, and have conducted their evaluation without external interference. Cost scoring utilizes a comparative formula dividing the lowest price proposal by the proposal being compared, multiplied by total possible points. All proposers must verify they have no active exclusions in the System for Award Management (SAM) and must certify compliance with applicable contract provisions provided as Exhibit A. Personnel identified in approved proposals cannot be changed without written agency approval, and contractor proposals must remain valid for 60 days after the proposal due date or until contract approval.
